Daily Health Tips: I’m A Breastfeeding Mother Who Just Developed A Painful Lump in My Breast. Could It Be Cancer?

Q: Hi Doc. I am a breastfeeding mom and l have discovered l have a painful lump in one of my breasts, Could it be cancer? Should l stop breastfeeding?

A: Painful lumps in the breasts of breastfeeding mums may be blocked milk ducts. Usually your milk ducts block if you’re making milk faster than you’re expressing it and indeed you should feel some relief every time you breastfeed from that breast…so stopping breastfeeding may just be making a bad matter already worse 😀

What causes this milk duct to block? It could be that your baby is not latching on well to the breast and thus not sucking enough from that breast. It could be that you don’t feel well and are not feeding baby often enough, or you decided to wean baby suddenly. Other causes include tight nursing bra, sleeping on your tummy, a nursing pump that’s not powerful enough, a blow to the breast or it could just be that a feed was skipped.
Solutions? Nurse your baby some more, ever so often….especially from that particular breast 😉 Yes I said that. Baby sucks hardest then and hopefully that strong sucking action would dislodge the plug. Otherwise try a nursing pump or just use your hands to express the milk.

Placing warm towels on the affected area may also help the breast milk flow faster and thus dislodge the plug.
Rest as much as possible, eat healthy (loads of fruits and veggies) and use pain relief, like Paracetamol, if needed.

If this condition is not handled immediately, it could lead to an infection of that duct (mastitis) such that the skin over that area feels hot to touch, reddens, mum feels feverish, ill and aches all over. This infection is usually treated with a course of antibiotics, which will be prescribed by your doctor.
So, if you try all the suggestions above and don’t improve, please see your doctor to confirm if this is an infection or indeed something else.

Daily Health Tips: Is It Bad To Let My 2-Year Old Son Swallow His Toothpaste?

Q: Good evening, Dr ketch. You are so wonderful. I enjoy your programme. My 2-year old boy swallows toothpaste when I am brushing his mouth. He always refuses to rinse his mouth too. Please what is the side effect of this?
A: Thanks for writing in about this very common mistake that a lot of mothers, unknowingly allow their children to make. So, you’re not alone 😀

The main reason why it’s not a good idea to swallow toothpaste is because of the fluoride in it. In small quantities, fluoride is great for teeth and prevents tooth decay by becoming part of the structure of developing teeth and by covering the surface of the teeth when it comes in contact with it. However, excessive amounts can lead to dental fluorosis, a situation in which there whitish splotches on the teeth and in really bad cases, black and brown staining of teeth with crumbling of the enamel. Other conditions associated with too much fluoride include, skin rashes and even fluoride poisoning which can show up as stomach pain, nausea, vomiting etc.

How do you stop this from happening?

First you have to make sure that you put only a small amount of paste on the toothbrush (pea-size…like the size of a corn seed). This reduces the amount of foam formed, and thus reduces the amount of fluoride the child can swallow.

Go for children’s tooth paste as they use smaller amounts of fluoride
Then be sure to supervise your child’s brushing from 2 years (when they able to spit) to 6 years to ensure that they brush well and that they spit out. You must also ensure that the mouth is rinsed, otherwise all that fluoride will be swallowed.

Remember that caring for your baby’s teeth starts even before the first tooth sprouts. At this time, a wet wash cloth wrapped round your finger does a great job of cleaning your baby’s mouth and gum.
As the teeth sprout, use a baby tooth brush and ‘barely-there’ (teensy-weensy amount of) toothpaste to clean baby’s teeth.

Daily Health Tips: How Do I Lose My Tummy Bulge After Having A C Section…Girdle vs Healthy Lifestyle.

Q: God bless you doc, even generations to come honour you already. Please, I hope you answer this because I’ve sent one you’ve not answered. I’ve had two caesarian sections now. And my tummy is very big because we were advised against tummy heating. Now I ask will it ever come down or what could I do? My first CS was 2010 and the other was April this year. And how early could I use “GIRDLE” God bless you, doc. Love you

A: Amen and Amen to all your prayers and I love you right back! 😀

Now as to tummy reduction after Caesarian Section, I wish it were as simple as a girdle 😀 As I always say, there’s no quick fix to these issues. You will have to go the good nutrition and exercise route (as soon as your doctor gives you the okay). There are some post natal binders or belts that are thought to be helpful but really, there’s no conclusive proof that they help your womb resume its normal position. In fact, used too early, it may rub against your incision site and interfere with healing.

So, the solution is a careful exercise and diet plan. For any kind of delivery, don’t start on any exercise regimen without checking with your doctor first. You can start off with pelvic floor exercises as soon as you feel up to it especially if you’re leaking urine when you cough or laugh. To locate your pelvic floor muscles, try holding your urine when you feel pressed. If you are successful, then you have located the muscles you need. Then empty your bladder and lie on the floor. Contract those same muscles and hold the contraction for about 5 seconds and then relax it for about 5 seconds. Gradually build this up to holding the contraction for 10 seconds. Repeat this about 3 times a day. Once you no longer leak, you’re probably ready for your exercises. As soon as the doctor gives you the all clear, you can take gentle walks, do simple stretches etc. Start small and scale up 🙂

As for diet, fruits, veggies, complex carbs and proteins are the way to go. The first rule of diet is portion control, reduce your plate (or bowl) size 😀 Then fill half the plate with veggies, a quarter with complex carbs like whole grains, oatmeal, local rice etc and the last quarter with protein eg fish and poultry.

Daily Health Tips: How Do I Stop Snoring?

Q: I need your advice on how I can sleep without snoring. It makes me not to have sound sleep. I will do anything you suggest as long as it will stop. Thanks for your care.

A: Snoring can be a problem as it can disturb sleep for you, your partner and perhaps even your neighbours 😀 The snoring sound is produced when breathing is partially blocked or obstructed as the aiways relax and then narrow while one sleeps.

It is possible to stop snoring through lifestyle modifications and they include:
• Losing weight. If one is overweight, the fatty tissues around the neck can squeeze the airway and this makes it difficult for air to flow freely.

• Sleeping on your back! I keep trying to get my daughter to do this and I’ve failed woefully so far 😀 But this may help as it helps keep your tongue out of your airway. It certainly helps my daughter when I can get her to stay in that position long enough to sleep 😀

• Stop smoking as this irritates your throat and can cause swelling. Again, this prevents the free flow of air.

• You know how you feel alcohol makes one relax? Well, they kinda do the same thing to the muscles of the throat 😀 This makes them collapse easily, reducing the airway. So, try and avoid this just before bedtime.

• Exercising often prevents you from being overweight and prevents those fatty tissues from squeezing your airways shut 😉

I hope these tips help. If the snoring persists, please see your doctor. He may suggest some anti-snoring devices or consider some other diagnosis like sleep apnea (in which one wakes up gasping for breath as the airways close up often during the night).

Daily Health Tips: My Husband Cheats. How Do I Prevent STIs?

Q: My husband is the “CHEAT” type who doesn’t care about the type of girl friend he goes to bed with. But on my side he deflowered me and I have never had any other man besides him. I have 2 children for him. How can I protect myself against STIs?

A: Wow! This is a tough question…especially in our traditional African society where the whole focus is on having women remain in their marital homes no matter what happens, whether you’re being beaten to death, getting ready to be infected by STIs or HIV etc.

Now, before anyone thinks of crucifying me, please note:
1) I’m a Christian
2) I love God and will do anything to ensure that each day I live to His glory 
3) I’m also a huge ‘respecter’ (is this even a real word?) of the marriage institution.
4) I also believe that marriage should be ‘for better or for worse and till death do you part’ and indeed, people should not start on that journey without being committed to going all the way with that journey.
I could go on and on with my beliefs on this matter, but that’s not what this post is about. It’s about the peculiar situation this woman finds herself in. I have seen many instances where women are infected by their husbands and indeed only get to know their HIV status when their husband’s status is diagnosed. And yes, I know that some men are also infected by their wives, but the odds are heavily stacked against the women in this. An UNFPA (2005) paper gives some stats:

About 50% of Senegalese women living with HIV had just one risk factor – living in a “monogamous” union.

About 90% of Indian women with HIV, said they were virgins before marriage and had remained faithful to their husbands

More than 30% of women in Mexico who were diagnosed with HIV only discovered their status after their husbands were diagnosed.

75% and 55% of women living with HIV in Thailand and Morocco respectively were infected by their husbands.
Finally, the paper went ahead to say that studies had shown that married women would rather risk being infected by HIV than confront their husband’s infidelity and and ask him to wear a condom. Indeed, only about 26% of women in 2 districts of Uganda said it was acceptable for a married woman to ask a husband to use a condom.

What should she do? There is really a simple ABC to this. 1) Abstain. I really don’t see the husband agreeing to this. 2) Be faithful. If the question is true, then this may not be very helpful. Being faithful to someone who’s not being faithful, does not help. 3) Using a condom. This should really be the rule of the game here, however, in a significant number of these situations that I’m privy to, the men in question would refuse to do this. Take a look at the stats above again. The figures today are not too far behind.

So, what options are available to a woman in this situation? In other climes, infidelity is cause for separation/divorce. We don’t follow this route too often in Africa. I’ve heard of women who would give their husbands condoms when they are travelling to ensure that when they sleep with other women, they play safe. That’s an option. Then there are women who pray that their men realize that they are putting themselves and their families at risk and hope that they use protection when sleeping with other women. That’s another option. Then there’s the option of keeping quiet and being hit with the news that one is HIV positive. The option that we don’t like to consider in this part of the world is that of insisting that hubby uses a condom. Some women have gone down this way and have broken bones and/or broken marriages to show for it.

The options, as I see them are between life and marriage. What would you choose? I’d really love you all to write in and give your thoughts on this matter and maybe, hopefully, we could be helping out this lady. By the way, she’s sent in this question three times and then written in to remind me further. Clearly, she needs help!

I will give one suggestion though, that as we give birth to our own children, we need to teach our children, male and female alike the importance of respect for each other. The males must be thought to respect the opposite sex and understand their responsibility to keep their wives safe from STIs etc. The females should receive education that empowers them to have a voice in issues that concern them and also understand their role in keeping the home disease-free too etc. We must correct the problems we see in the society with our own children. It starts from you and me. If a son knows that his sister is liable to be infected with HIV because her husband plays around, hopefully, he may not want to tow the same route, methinks 

Daily Health Tips: What Drug Can Help Me Get Pregnant?

Q: Good Morning Dr. Please, thank God for your life and for your family’s life. Please, I saw you on Wednesday on DSTV. Please, can you tell me the medicine to take when you want to get pregnant? I am 40 years old. Thanks and God bless you.

What drug to take? Well, if you’re planning on getting pregnant, it’s always a great idea to start taking pre-natal vitamins especially those that contain folate (folic acid). Generally though, eat healthy: lots of fruits and vegetables, exercise at least 150 minutes per week. Be sure to check with your doctor as to what exercise that you should be involved in and how to progress if you have been sedentary for a while.

On the other hand, if your question speaks to drugs that help to induce ovulation, then you need to see a good obstetrician/gynaecologist to take your medical history and guide you through this process based on your peculiarities. It’s never a great idea to go buy ovulation inducing drugs off the shelf and start taking them with no guidance. So, please be guided.

Daily Health Tips: Should I Pop Vitamins When I Am Pregnant?

Q: Please, is it advisable for a pregnant woman to take vitamins? If no, please what are the side effects on the mother or the baby? Thanks

A: Nutrition plays a huge role in pregnancy. Soon-to-be-mums need to eat healthy to ensure that they give their children a good start in life. However, sometimes, there may be a few holes to plug, in situations where our diet doesn’t quite satisfy all the nutritional needs. Pregnancy may be one such time.

Now, remember that pregnancy is also not the time to go popping different kinds of pills without a prescription. You don’t want anything that prevents your baby from developing normally.

Pre-natal vitamins are a ‘usual’ part of the process of pregnancy. They would usually contain, at least folate to help the proper development of the spine ( and prevent neural tube defects), calcium (to ensure that baby doesn’t depend on mummy’s store of calcium in her bones for its own supply) and iron (to help prevent shortage of blood).

So, speak with your doctor about what vitamins you would require. Remember to also ensure that you feed well to give you and your baby every advantage during this all important period of pregnancy.

Daily Health Tips: My Hubby is Rhesus Positive and I Am Rhesus Negative. Is My Baby At Risk?

Q: Good evening Dr, your tips are really helpful. I am O NEG while my hubby is O positive. I am AA and he is AS. I had an abortion before we got married though. Right now I am about 38wks pregnant now but my Doc said I should have taken an injection before getting pregnant because our blood groups are incompatible to avoid fresh still birth. I have been so scared. Since on 31st August, I’ve been feeling pains in my abdomen and waist. I don’t know if it is labour.

A: Hello dear, you’re technically ready to deliver and so the pains you’re having may very well be labour. Please visit the hospital to check it out…especially given that you have no experience to compare the pains with.
The fact that you’re AA and your husband is AS, is not really an issue and is not the reason why your doctor talked about ‘incompatibility’. With this genotype, every time you put to bed, you have a 50% chance of having a baby with AS genotype and a 50% chance of having one with AA genotype. The main issue is the Rhesus incompatibility…the fact that your husband is O negative and you’re O positive.

Usually in pregnancy, your blood and your baby’s blood do not mix. However, during the process of delivery (and some procedures during pregnancy), some degree of mixing of mother’s and baby’s blood happens. When this occurs, if your baby is O positive, your body starts to produce antibodies against your baby’s blood….literally. But, because your baby is being delivered or has been delivered, the antibodies have very little time to do any damage to the baby. They can only get rid of the small amount of baby’s red blood cells in your own body. So, baby number 1 is not a problem.

The challenge though is that these antibodies are now waiting for another opportunity to attack foreign blood cells. If you give birth to a baby that is O negative like you, this is not a problem. If, however, the next baby is O positive, those antibodies kick in and start to attack your baby’s red blood cells. It is for this reason that the Rhogam injection is given within 72 hours of delivery to ensure that your body does not mount the regular response of producing antibodies against Rhesus O positive blood. The injection deceives your body into thinking it has already produced antibodies, thereby protecting your baby as the particles of Rhogam cannot cross the placenta.

I hope in your case, that your first pregnancy involved a Rhesus negative baby…in which case the injection you didn’t take wasn’t needed anyway. Whatever be the case, be sure the injection is administered this time.

Don’t beat yourself up over the injection that you didn’t take. It’s past. Let’s focus on the present and be sure to prevent future issues like this. Your doctor already knows what complications are likely to be expected and I trust that he will have the right team in place to handle these as best he can.
