Daily Health Tips: Should A Pregnant Woman Fast?

Q: Good morning Dr, I missed my monthly period. I think I am pregnant lolzz (Happy me). I need your advice. Please, my church is embarking on 10 days praying and fasting. Should I fast? If yes, what time should I break my fasting? When is the right time to run a Pregnancy test?

A: Congratulations!!! 😉 I join my faith with yours and…..fingers crossed!

Should you fast? There is contradicting medical evidence as to how safe (or not) it is to fast while pregnant especially during the first trimester. So, this is a question I believe you should ask your doctor who has your medical history and is better able to speak to how you can/will cope or not.

Generally, during a fast, the body truly enters a fasting mode after it has absorbed the nutrients from the last meal. This takes about 8 hours. Its next source of energy is the glucose in the liver and muscles. If fasting continues, then it moves to the next source…which is fat. After it has depleted this source, then it goes into real starvation mode and begins to break down proteins!

I presume that you will be breaking your fast daily and so you will hardly be candidate for the starvation mode! Let your doctor guide you on the issue of whether or not to fast and how many hours to do so, if you participate. Ultimately, the greater goal is to have you and baby well enough to praise God.

A pregnancy test reads positive about 6 to 12 days after implantation has occurred. There are two types of pregnancy test (PT): the blood PT which is more sensitive and the urine PT.

Are You A New Mum? This Is For You!

Plain warm water is sufficient but some people add salt to the water which is soothing. Your doctor may also suggest other things he/she may want you to add to the water.

Daily Health Tips: How Can I Protect Myself From The Germs I Am Exposed To At Work?

Q: Hello Dr, my job is waste collection. I work with ministry of environment. What can I take to prevent infections from the constant inhaling of the waste I am exposed to?

A: Thanks for writing in. This is a very important question given that you are exposed to these conditions on a daily basis. This also important for the rest of us as we go about cleaning our environment especially over the weekends.

To start off, ensure that you are dressed appropriately with the right long sleeved overalls. These overalls are usually reflective and if they are not be sure to ask for a reflective vest. This makes you visible to other drivers and even the driver of your own garbage truck. Accidents have been known to occur where a person collecting waste is run over by their own duty truck! Ensure that you also wear gloves and heavy boots.

The gloves protect against cuts from shards of glass and other sharps that may be in the bin liners (dustbin bags). It can also protect from potentially dangerous and corrosive substances like acid etc.

By the way, even when cleaning the home, please use gloves to clean areas like the toilets. Yes, I know that it is your poo…but guess what there are loads of germs in there! So, wear gloves to scrub and then be sure to scrub your hands thoroughly afterwards 😀

Ensure that you lift with both hands after testing to feel the weight of the bag/garbage. You may want to invest in a back support body belt too. This will help prevent back strains. There are also potential injuries from slipping on slippery surfaces especially when it rains. To prevent this, ensure that you wear the right heavy non-slip boots.

Earplugs (disposable and re-useable) should be used to protect the ears from the continuous noise of the truck. Disposable masks may also be used to reduce the impact of the smell of garbage.

Given the potential for being cut by sharps, please ensure that you get vaccinated for tetanus and hepatitis. Eat healthy ensuring that your diet has lots of colour (fruits and vegetables). This helps build up your immune system ensuring that your body is ever ready to fight infections.

Studies show that though people are prone to infections due to the very nature of this job, the most problems encountered really speak to injuries obtained due to lack of use of personal protective clothing. So, play safe and dress safe 🙂

Q: I am a teenager of 18 years 8 months, my height is 171 cm and weight is 45 kg. My BMI is 15.4. I am underweight and I really, really need to add weight before April – June 2017 for a BMI screening. Please Doctor what should I do?

Q: Thanks for writing in, dear.

Your BMI is a quick way to tell whether or not you might be underweight or overweight, and what health problems you might have or develop because of your weight.

It is actually a measure of weight to height. It is calculated by dividing your weight in kg by your height squared in meters (that is height x height). Eg, if one weighs is 80kg and is 1.6m tall, the person’s BMI is 31.25. Different BMI readings have different meanings.

If you’re under age 21, your age and gender need to be factored into your BMI to account for the different speeds at which guys and girls develop.

If your BMI is less than 18.5, you are in the underweight category. Talk to your doctor to find out if your weight is a symptom of a medical problem. A registered dietitian can help you learn about healthy eating.

If your BMI is between 19 and 24.9, you are in the recommended weight range for your height. But your health may still be at risk if you are not getting regular physical activity and practicing healthy eating. So, don’t start rejoicing just yet

If your BMI is 25 to 29.9, you are in the overweight category. This may or may not be unhealthy, depending on some other things, like your waist size and other health problems you may have.

If your BMI is 30 or higher, you’re in the obese category. You may need to lose weight and change your eating and activity habits to get healthy and stay healthy

Apart from your BMI, your waist measurement also plays a significant role in your health status.

It’s interesting that being underweight comes with its own problems…much like being overweight. People who are underweight are prone to having nutritional deficiencies like brittle bones because of lack of calcium. This leaves them open to bone fractures. They are also prone to diseases as they have weak immune systems. They are always tired, hardly having any energy to do stuff and they also have menstrual irregularities. If they haven’t started having periods, it’s further delayed and if they have started having periods, then it becomes irregular.

The first thing would be to find out why you are underweight. Have been ill, recently? Have you been snacking more than you’ve been eating proper food? Are you stressed? Are you avoiding food so that you don’t add weight?

Generally though, to add weight in a healthy way:

Focus on calorie-dense but nutritious foods like eggs, nuts and milk. Full cream milk is definitely an option, especially if you don’t have a high cholesterol level.

Eat often…6 ‘small’ meals per day is not out of place. Starchy carbs are great too (local rice, potatoes, oats, whole grain/wheat bread etc). Some exercise before meal times is a great and healthy way to work up an appetite. Taking some fruits as appetizer before mealtimes also helps stimulate an appetite. Smoothies are also great breakfast and snack options. Dried fruits and nuts can be added to cereals, salads and even yoghurt. Try not to drink too much water before or during a meal to ensure you have enough space for the main event which is the food  😀

Remember that it only takes a little effort to pile on the weight…a second on the lips and a lifetime on the hips, aye 😀 So focus on complex carbs and lay off the sugar-filled calories and pastries (these are empty calories which create more problems than they solve) and remember that nuts should be taken by the handful, dairy should not exceed 4 glasses per day, oil should be used sparingly etc. Indeed, moderation in everything…as usual.

If none of these help, please be sure to see your doctor to ensure that this is not the symptom of another problem like an eating disorder.

Daily Health Tips: Bad Breath In Children And Adults

Q2: Good morning Doc. Please I need you to advise my friend who’s suffering from bad breath or mouth odour. He has visited the dentist and has used mouth wash and other things yet to no avail. He’s very worried about all these. Please, what do you advise him to use or do? Thanks ma’am

Q4: A very good day to you ma’am. Please ma’am, my four year old daughter has bad breath. I’ve tried everything hygienically possible to curtail the bad breath but after some minutes, it comes back. Please, I need urgent help. Thank you ma’am and God bless you.
 
A: Thanks to you all for writing in.
I will start with the last question about bad breath on children…
 
Bad breath at any age is traumatic but in a young child, it often leaves the parents wondering what they are doing wrong
Improper brushing of teeth is always an issue. People argue that they brush their teeth and their children’s teeth properly, but this may not be completely true. Do you brush your teeth for at least 2 minutes, paying attention to every section? Do you brush your tongue? Do you floss? Remember that food particles between teeth can interact with bacteria that live normally in the mouth and lead to stinking mouths…same for food debris at the gum line and tonsils. And then, when a baby has a congested nose and breaths with his/her mouth, the dry conditions enable the bacteria to thrive.
Tonsilitis, finger sucking by babies, tooth cavities and buildup of tartar on teeth can all cause bad breath.
 
What can you do as a parent? Get involved. Help your baby with brushing the teeth to teach him the right technique. (When you say, he brushes twice a day, I hope this doesn’t mean he’s doing it without supervision?) Brushing times can become fun bonding times for mummy and her son or for daddy and son. Sing nice fun songs and brush your own teeth at the same time too
 
Remember that oral care (care of the mouth) should start even before the first tooth erupts. Regularly wipe your baby’s mouth and gum with a wet flannel and as soon as the first tooth/teeth erupt, start brushing. Use tooth paste that does not contain fluoride until your baby can spit. What tooth paste does not contain fluoride? Ask at your regular supermarket or simply pick up the packs of the different tooth pastes and look for one that does not contain fluoride
 
 Use a tiny amount of paste at this initial time (about the size of a grain of rice), then graduate to a pea sized amount and tooth paste that contains fluoride, when the child can spit out.
 
Remember the rule of twos: Take your baby to see the dentist twice a year, help him and teach him to brush his teeth at least twice a day (three if you can manage it, after every meal) and be sure to ensure that he spends at least 2 minutes brushing his teeth. A simple way of doing this is mentally dividing the mouth/teeth into four quadrants/sections. Then spend at least 30 seconds on each side, making sure you brush every surface of the teeth.
 
Then be sure not to forget the tongue. Pay attention to every side and use a soft bristled brush to do a good job. There are tongue scrapers which can be purchased alone or tooth brushes which come with bristles at the back for brushing the tongue.
 
Stay off alcoholic mouth washes for your baby as the alcohol just worsens the dryness in the mouth, And remember that mouth washes are just temporary fixes. They only mask the bad breath and not treat it.
 
Encourage your baby to drink water regularly, especially if they have dry mouths and they’ve got to scrub their hands regularly too, if they suck their fingers/thumb.
 
These should be sufficient to stop the bad breath. If they don’t however, please see your doctor to rule out other causes of bad breath which cavities in the teeth etc
 
And for adults…..
Bad breath, also called halitosis, may be as a result of poor hygiene habits, eating strong smelling foods or could be a sign of other medical conditions.
If adequate dental hygiene is not maintained, food particles can remain in our mouths, encouraging bacterial growth bacteria in our mouths can break down food particles and raise a serious stink. Proper dental hygiene habits includes:
Brushing your teeth twice a day (this sounds like a ‘no-brainer’ but the right technique is important. Mentally divide your mouth into four quadrants and spend at least 30 seconds on each quadrant. This way, you spend a minimum of 2 minutes brushing)
Brushing your tongue (if you finish brushing and you can still see all that whitish residue on your tongue, you haven’t done a good job. Some of the smell comes from there. Some tooth brushes come with a tongue scraper at the back of the brush…use that. If not, buy one or just use your brush)
Flossing your teeth everyday. Ask for this in your local super markets. This gets rid of the food particles between your teeth where your tooth brush can’t reach.
Using a mouth wash can also help the prevention of mouth odour.
Eating strong smelling foods like onions and garlic could also cause mouth odour which may not be taken care of by brushing. This is because these ‘foods’ are absorbed into the blood stream and excreted through the lungs. So, the smell will persist until excretion is complete. The small can be masked with mouth wash etc
Disease conditions that can lead to bad breath include mouth infections, Diabetes Mellitus, sinus infection etc.
Other conditions like use of tobacco (chewing or smoking), some drugs and dry mouth (which is the reason for morning breath :D) are also culprits.
Prevention is focused on the tips given above and ensuring that you visit your dentist at least twice a year, brushing at least twice a day (if not after every meal) with a fluoride tooth paste (remember that fluoride is not recommended for children less than 2-3 years and even then, only a pea size is recommended), change your tooth brush once every three months and drink lots of water.
Let’s keep the pearls shining and the breath smelling really fresh.

I asked that question during one of my shows and most people didn’t want to be the one that causes offence by telling someone that they have bad breath. However, most people also wanted to be told if they had bad breath. So, who will bell the cat? :d
