So, it’s a great idea that you wish to lose weight but how you choose to lose it is the problem. I’m also not sure how you arrived at the ‘preferred’ weight of 70kg. Truth is going by your weight you will need to get down to at least 54kg for you to be in the ideal BMI range! But that’s not even the issue now. The main challenge for me is that trying to lose about 22% of your weight (20kg) in 6 weeks. That is actually dangerous. To lose weight in a healthy way, you should not be losing more than 0.5kg to 1kg per week. What you’re proposing comes to about 3.3kg per week! This is not just unhealthy but not sustainable. What you’re likely to be losing is water weight or losing some of your muscle mass…but you’re really not losing much of the fat. Remember that it took you a while to get here and so, it’s definitely going to take a while to undo the damage.

Can you lose weight without exercise? Absolutely, but if you exercise, you lose weight more efficiently and actually faster. This is because with exercise, your rate of metabolism increases and so your body efficiently breaks down your food. Remember that how much weight you have is a balance between how many calories you eat everyday and how much of it you burn through exercise/activity. If the calories taken in and the calories being burnt are the same, the person’s weight remains stable. If the loss is more than the intake, the person loses weight and if the intake is more than the loss, then the person gains weight.
So, there are two key factors here…food and exercise.

For food, start by reducing your portion size. Divide your plate into two. Fill one portion with veggies/fruits and then divide the other half into two. Fill one half with lean protein like fish, chicken or trim red meat, if you must and in the last portion, put your carb. Focus here should be on complex carbs like local/wild/brown rice, beans, whole wheat bread, oatmeal etc.

Then eat slowly. It takes about 15 minutes for the signal confirming fullness to get to the brain and so when you eat too fast, you don’t have time to appreciate this signal before you’re stuffed. In fact, this is worse when you’re eating while watching TV…this usually translates to mindless munching of food even when you’re full.

Daily Health Tips: Nipple Pain

Q: Good morning Dr, please the tip of my breasts have been hurting. Please note that I made love with my husband during the unsafe period. What is wrong with me?

A: I’ve received a couple of questions on nipple pain. There are many different causes of nipple pain and they include:
• Given the fact that sexual intercourse happened just before the complaint of nipple pain, it may very well be that this led to the pain. Vigorous rubbing of the nipples can lead to pain. This would usually heal without incident and applying a moisturizer will help the process.
• Friction: In this case, not wearing a bra or wearing a poorly fitted bra during exercise routines may lead to rubbing of the nipples against the fabric and consequent soreness/pain. Wearing a proper
• Sensitivity to the weather (hot or cold), certain fabrics, detergents or even creams/lotions may lead to nipple pain. If this is caused by specific detergents or creams, you may have to change them.
• Hormonal changes as can occur close to a woman’s menstrual period. This pain will usually disappear after a few days.
• Pregnancy and breastfeeding can also lead to this condition. In pregnancy, the pregnancy hormones lead to the nipples being extra sensitive and during breastfeeding, the baby may not latch on well to the nipple, there could be blocked ducts or mastitis or breast abscess etc leading to pain. During pregnancy, be sure to choose bras made with fabric that can ‘breath’ like cotton. And during breastfeeding, be sure to get help with making sure you know how to lput baby to the breast to prevent sore nipples etc.
• Cancer (Paget’s disease of the breast) can also be a cause of nipple pain.

So, where in doubt…especially if you have tried to rule out the causes above, please see your doctor.

Eating Right!

If you’re trying to lose weight, maintain your weight or even just saty healthy, eating right is key.

Daily Health Tips: Storage Of Breast Milk Re-Visited

Dear family, thank you so much for your kind thoughts and wishes to me yesterday. You overwhelmed me with your love. I’m not sure how many of you got around to answering the question, ‘what are you thankful for?’ but I’m certainly thankful for the love I felt from all of you yesterday.

May your joy be full always!

And now to today’s question…

Q: How can I store expressed breast milk?

A: Expressed breast milk has to be handled in a very hygienic way. It is important that it does not form the means by which germs find their way into your baby’s body so utmost care must be taken.

What should you do? I reproduce a post I had made on this a while back;

To express milk for your baby, first, you have to be committed to be hygienic because you don’t want to introduce germs into your baby, do you? Always scrub your hands with soap and water whether you are using the breast pump or your hands.

Having expressed the milk, when stored at room temperature, be sure to use up the milk within 6 hours.

When stored in the fridge,
• Ensure that the temperature in the refrigerator is below 5 degrees centigrade.
• Do not store on the door of the fridge. Please store on the shelf towards the back of the refrigerator where it’s cooler
• At this temperature, you can store milk for up to 5 days.

When stored in the ice compartment of the refrigerator, you can store milk up to 2 weeks

When stored in the deep freezer, you can store milk up to 6 months.

To thaw frozen milk, please put it in the fridge. When it’s thawed, you may feed your baby with it but if your baby prefers it warm, you can run some warm water over the bottle until it’s just right. Do not microwave it as this may create some hot spots in the bottle which may cause scalding of your baby.

Once thawed, please don’t re-freeze the milk as this might encourage the growth of micro-organisms.

Make sure that the container used for storing milk is sterilized to prevent any potential introduction of germs into the milk. Use small containers that hold just about the amount that your baby finishes per feed.

If you express a lot of milk, be sure to label the different containers and use the oldest pack first. Please stick with the duration of storage guideline we’ve given.

Now, it goes without saying that you should not store milk close to contaminants like meat, fish or even strong smelling foods like onions 😉

Daily Health Tips: How Do I Stop Breastfeeding My Baby?

Q: Hello, thanks a million for all your educative posts. Dr. I did exclusive breast feeding for my baby for 6 months. Now she is 16 months old. I want to stop breast feeding. Please tell me to go about it.

A: Breastfeeding is really an intimate activity for mum and baby and it is not strange to find both of them feeling some sense of loss as this ‘bonding’ activity comes to an end…even when it’s mummy suggesting the change 😀 So, it is important that mum finds time to still bond with baby like frequent cuddles, keeping baby close to you, frequent eye contact etc. Generally re-assure your baby that the fact that this activity is coming to an end does not mean you love him or her any less. Be sure not to introduce this when your baby is going through any major changes like moving house etc That might convince baby you’ve moved on to other stuff 🙂

Then get to it! 😀 How? You can stop gradually over a period of time. Replace some feeds during the day with other foods or even milk (just not from you). Usually the first and last feeds of the day are the most difficult to replace or give up as they are your baby’s sorta emotional anchors 😉 But keep at it over a period of weeks and the breast milk production will gradually ‘dry up’.

The other option is to just stop putting baby to the breast. If your baby feels it’s also a good idea to stop, then you’re good. If not, you may have to be very creative with means and ways of distracting him/her.

In both situations, you may find your breasts engorged. If you’re stopping gradually, you may want to express some milk (don’t express until until the breast feels empty) and store in the fridge. You could put that in a sippy cup and feed baby later.

If you’re stopping immediately, get the right support for your breasts.

In the good old days, mothers would tell their daughters to tie a wrapper tightly across their breasts. This helps. In fact, I believe that this is still being done by lots of people 😀

If you’re more jet age, though wear a tight supportive bra. For the pain, take analgesics and also apply cold packs to your breasts.
Try not to stimulate the nipples during this period. This encourages milk production. Hubby should kindly take note too.

Today is World Hand Hygiene Day!

The World Health Organization calls on health workers to ‘Wash Hands and Save Lives’ I believe the importance of hand hygiene by all, especially health workers was clearly demonstrated during the Ebola scare. Just a fleeting touch or contact with a contaminated surface was enough to cause an infection!

So, since the Ebola scare, what are the key learning points you have taken away and still practice with regards to hand hygiene? If you have children, what important lessons have you taught them? What does hand hygiene mean to you?
