Q: Good evening Dr, God bless you more for the good works. Please is it possible for someone that gave birth 6 years ago to go back to the doctor because of the cut they gave her for them to sew to back. She said that her body is too open, some times she hear some sound from her virgin. Please she need your advice. Thank you Ma’am.
 
A: Why do you think that the cut needs to be sewn again? Is that because you feel loose down there or because you have experienced the quite embarrassing vaginal farts? 😀 Vaginal farts are much more common than you may think. However, many women would rather not talk about it because, it may be somewhat embarrassing! Vaginal farts occur when the vaginal walls expand, pulling in lots of air, and then collapse back to release this air. They are usually odorless, in contrast to anal farts. It can happen with exercise, sex, and with sudden movements like getting out of a chair.
 
The treatment for vaginal farts is really the same as for feeling loose ‘down there’ after you just had a baby. This treatment is the usual suspect…pelvic floor exercise called Kegel exercise.
 
This is done by trying to hold your vagina together, tightening it like trying to hold urine. Hold these contractions for 10 seconds, then release for another 10 seconds. This should be done several times a day .You can do these exercises anywhere; while waiting on a queue, working at your desk or while watching TV. It may take 6-12 weeks of exercise to see results. These exercises can strengthen the pelvic wall muscles and help keep the vaginal walls from expanding. These exercises provide other benefits like facilitating childbirth, enhancing orgasm, and decreasing the chances of having a leaky bladder later in life.
 
Try keeping your legs together as much as possible, and also avoid using tampons when you are not menstruating.
 
The best advice is to think of vaginal farting as natural and fun instead of embarrassing 🙂 However, please see your doctor if the Kegel exercises do not help and you feel the same way.

Daily Health Tips: Why Do I Have Breast Pain?

Q: Good Afternoon Dr. Please I Have Pain In My Breast. What Should I Do?

A: Breast pain can be experienced as a burning or stabbing pain or it could be felt as soreness or heaviness. This usually occurs in the outer portion of the breast and may extend to the underarms and sometimes, all the way down the arms. Breast pain is commonly not a sign of cancer.

Breast pain could be cyclical, in which case, it is linked to menstrual cycle or non-cyclical, in which case it is not linked to menstrual cycle.

Cyclical breast pain occurs at about the same time every month, starting about 2 to 3 days before the menstruation starts and improving after the period. The intensity of the pain varies during the period. This pain is thought to be related the effect of hormones and so women on hormone replacement therapy after menopause may experience this breast pain too.

It may also be due to medications, breast abscess (collection of pus in the breast), non-cancerous breast lumps .

If you have breast pain, it’s a good idea to have your doctor check it out especially if:

  • The pain is continuous over a couple of weeks
  • The pain gets worse
  • The pain prevents daily activities
  • The pain is consistent in a particular area.

A couple of exams like clinical breast examination, mammogram, ultrasound and breast biopsy will help make a definitive diagnosis.

What Is Healthy About Healthy Oils?

We talk about healthy oils: Olive oils, canola oils etc. But really, even these contain pure fat with about 120 calories in each tablespoonful of olive oil, be it light or extra light. To put this in context, consider how much time it takes to burn off 100 calories at the gym and the relative ease with which a tablespoon of oil goes onto our food. In the Johnny Walker chapter, we talk about walking 10,000 steps per day. If you do those 10,000 steps in a day, you burn between 300 to 400 calories per day and less if you walk mainly flat surfaces! And yet, with one spoon of oil, more than a quarter of all that ‘amazing hard work’ has gone down the drain! SO remember your screams of ‘oh no! 10,000!’ and compare that with the calories burnt and then how much you gained by just adding a drop more oil. If this doesn’t convince you to go easy on the oils, I don’t know what else will!
 
If you come from the Yoruba tribe (please be sure not to tell them I said this :|), then traditionally, cooking involves ladles and ladles of oils like palm oil (which has a high content of saturated fats associated with high cholesterol levels). I remember the first time I was exposed to stew from the Yoruba race, it had so much oil that I couldn’t see the tomatoes! Being a ‘kobo kobo’ girl who was born into a race where we live a diet of vegetables (in as raw a state as possible) and little oil, that was a culture shock and I never quite got used to it (Please don’t let my in-laws know that all that time I pretended to love those stews, I was actually just suffering through them ;)). It was almost as great a culture shock as learning to kneel down to greet. I developed waist pain the first time we travelled to the village what with all that kneeling which my ‘okoro ‘knees were not used to Again I digress!

Daily Health Tips: Why Are My Nipples Leaking Water?

Q: Good evening. Please, my nipples are removing water. At the same time, my menses has passed and I have slept with my fiancée for up to two months.

A: There are different issues that can cause nipple discharge. The discharge can be bloody, milky, clear etc. The colour may not tell us much as to whether the discharge is normal or abnormal.

Causes of breast discharge include:
• Pregnancy
• Breastfeeding: The release of milk may also persist for a while even when breastfeeding has been stopped.
• Stimulation of the nipples during sexual foreplay or even pressing the nipple by people who are worried about the nipple discharge or friction between fabric and nipple can lead to nipple discharge.
• Breast abscess or infection
• Hormone imbalance
• Injury to the breast
• Some medications
• Cancer of the breast

To determine cause, the doctor may do a scan, take tissue sample to examine (biopsy) etc
Treatment depends on cause. If due to pregnancy or breastfeeding, it will eventually stop. If due to stimulation, the discharge will continue as long as the stimulation is present. If due to injury, it will also stop with healing. If the discharge is due to medication, stopping the medication is sufficient to bring this to a stop.

Bloody discharge, discharge from one breast only and discharge that happens spontaneously without the breast being touched or pressed is likely to be abnormal. These should be checked out as soon as possible in the hospital.

If the discharge is present during a menstrual period and persists into the next cycle, please see your doctor too.

Daily Health Tips:Post-Pregnancy Bleeding After Sexual Intercourse

Q: I delivered my baby 3 months ago. The first time hubby made love to me I bled 5-6days as if I was menstruating, This has happened many times. Please what is the cause and what should I do for this to stop?

A: Congratulations on your new baby!

Usually, after having a baby, the level of the hormone estrogen is quite low. This lasts for about 4 to 6 weeks after having baby. If you’re breastfeeding your baby, however, this low estrogen level would persist for as long as you nurse our baby. The absence of estrogen contributes to vaginal dryness. The dryness, of course, makes sexual intercourse that much more painful at this time when your body is struggling to get back into the groove and your mind is struggling with trying not to remember the diapers you have to change and laundry to wash! Not exactly the most romantic turn of events, aye? 😀

The main consequence of vaginal dryness is the fact that it affects sex life such that these women experience significant pain and sometimes, bleeding. Other problems that vaginal dryness could cause include soreness, itching and burning around the vaginal area, recurrent urinary tract infections and frequency of urinating etc. However, you should also get this bleeding checked out by your doctor to rule out any other cause of bleeding there.

As you start to breastfeed your baby less frequently, the dryness should ease off. If the vaginal dryness and subsequent bleeding is due to rushed foreplay, there should be a conscious effort to slow down allow enough time for foreplay and sometimes, discussion with the partner may be helpful. Avoid heavily perfumed products like sanitary towels, tissues etc which may lead to irritation of the vagina and subsequent dryness etc.

Daily Health Tips: Pregnancy And Menstruation

Q: Good day Dr Ketch. May God crown all your efforts as you lecture people. Please, can a lady be menstruating and be pregnant at the same time?
 
A: People do not menstruate when pregnant. Remember that menstruation happens when fertilization of eggs released during ovulation by sperm does not occur. And so, if fertilization and pregnancy occur, menstruation is ruled out.
 
Bleeding can occur in early pregnancy though, close to or about the same time as the menstruation would and thus be mistaken for a period. This bleeding could be due to implantation as the fertilized egg implants itself in the uterus (womb). Other causes of bleeding in early pregnancy include ectopic pregnancy, miscarriage and infections.
 
Bleeding in pregnancy may not necessarily be an emergency but because there are other causes of these episodes of bleeding, which are potentially life threatening like ectopic pregnancy, bleeding in pregnancy should always be checked out in the hospital. Better safe than sorry 🙂

Daily Health Tips: Withdrawal Method And Secondary Infertility

Q: Good evening ma’am! Please, I had two kids through CS and the second child will be 3yrs in July and I used withdrawal method all through. Since last year July, I started trying for conception all to no avail. I went for scan and I was told there was no problem. But up till now, there’s no baby to show for it. I am worried and I am 36yrs! Please, Doc help me.

A: Withdrawal method involves pulling the male organ out of the vagina just before ejaculation. This method can protect against pregnancy if used well but does not protect against STIs. It has been argued in some quarters that the pre-ejaculate (a lubricating fluid) may contain some sperm cells and so even if your partner is able to withdraw before ejaculation, the harm would have been done. However, the sperm in the pre-ejaculate is not there at the time the fluid is produced, but is sperm from previous ejaculations that was probably left in the urethra. It is believed that urinating and cleaning the tip of the penis before sexual intercourse helps to clear the urethra of the sperm left from previous ejaculations. Congratulations on having been able to prevent pregnancy using this method! 😀

Secondary infertility is very common, but not often talked about. Even when women present themselves to the hospital, they don’t come out straight to let the doctor know what actually brought them to hospital, and rather list a series of symptoms most often, unconnected to their primary (main) concerns. Secondary infertility is the inability to become pregnant, or carry a pregnancy to term following the birth of one or more children. There however has to be frequent (at least three times a week), unprotected intercourse for a period of at least one year (six months for women older than 35), for this definition to be complete.

 

The causes include impaired sperm production (quantity and/or quality), erectile dysfunction, or new disease conditions in males. In females the causes include tubal damage (that is damage to the tubes from infections or adhesions in which case certain body surfaces stick together following surgery), ovulation problems (menstrual abnormalities), uterine conditions (infections from complications during a prior delivery, retained placenta, or a miscarriage which wasn’t properly cared for), complications from previous pregnancy(excessive bleeding following delivery) and changes in you or your partners’ risk factors; like changes in age (increasing age), weight, smoking and use of certain medications or disease conditions.

 

Oftentimes, no reason can be found for the infertility and this is even more nerve wracking, because nobody has an idea how long it’s going to last or what treatments are going to work best. Joining support groups and/or counseling sessions may be helpful during this period of waiting.

 

Prevention of infertility is targeted at the risk factors. Quit smoking, reduce alcohol consumption, exercise moderately and keep your weight within normal limits.

 

Find a hospital with a good Obstetrics and Gynaecology specialist. The closest teaching hospital to you may be a good place to start your search. He will examine you properly and advice on the best course of treatment for you.

Daily Health Tips: Chronic Post-Surgical Pain

Q: Good day Dr. My mummy had surgical operation as of 2004 and since then she have being complaining of pains in that area . Please Dr what do you advise she will do because am worried about that. Thanks
 
A: Pain that persists or keeps coming back for up to three or more months after surgery is known as chronic post-surgical pain. This is sometimes difficult to isolate as usually the surgery would have been carried to deal with a painful condition and so the question would now be: is this the old pain or a new one? If it is a new one, is it related to the surgery?
 
Features that point to post-surgical pain include:
• Pain that starts after surgery
• Pain that remains for more than 3 months
• Pain that cannot be linked to the original painful condition
• Other painful conditions are not present and cannot be linked to the chronic pain
 
The chronic pain may result from irritation of nerves during surgery. People at risk include those who already have other chronic pain conditions, people who did not have the initial; acute pain from the surgery adequately treated and controlled, people who were overly anxious about their surgery etc
 
Treatment of chronic post-surgical pain depends on the mechanism that is behind the pain. Treatment options include use of medications (pain killers, anti-convulsant drugs etc), injections, nerve stimulation technique known as Trans-cutaneous Electrical Nerve Stimulation and/or additional psychological help.
 
So, take mummy in to see a doctor who will examine her properly and make a diagnosis. Thereafter, he will decide on the appropriate treatment options.

Daily Health Tips: Pain And Blood In My Throat

Q: Dr please I am having a problem. My throat hurts. If I force myself I will see something like blood. What should I do?
 
A: Pain or irritation in the throat which may worsen when you swallow, is usually sore throat.
 
The most common cause of sore throat is viral infection like common cold. This type of sore throat resolves without the use of any medication. As any ardent follower of this page should know by now, viral infections are self-limiting and so they will run their course and stop with or without any treatment. Other causes of sore throat include bacterial infection, irritants (like tobacco smoke), muscle strain (from screaming), allergies etc
 
Symptoms are typically dry throat, difficulty with swallowing, painful sensation in the throat, hoarse or muffled voice, swollen tonsils on looking into the mouth etc
 
Treatment for sore throat depends on the cause. However, since most sore throats are caused by viruses, treatment is mainly symptomatic (that means, addressing the symptoms) and includes:
Rest: This helps rest your voice and improve your immunity
Drink lots of fluids to prevent dehydration and gargle with salt water. Please do not swallow the salt water.
 
Use drugs like ibuprofen or paracetamol for pain and fever.
Use antibiotics ONLY if prescribed by your doctor who believes the cause of the sore throat is a bacterial infection. Suspicion of bacterial infection is made by the doctor if the patient has tender lumps (lymph nodes) on the neck, has fever, has no cough and has pus on the tonsils. If patient has three of these symptoms, the doctor may decide on antibiotics.
 
Prevention is the same as that for preventing infections generally. Practice good hygiene in all aspects (personal hygiene, hygiene at home, pet hygiene and food hygiene) and this involves:
 
Hand washing or using hand sanitizers when hand washing is not immediately possible, coughing or sneezing into the crook of the elbow, regularly cleaning ‘forgotten’ surfaces like remote controls, light switches, knobs of cookers etc all help to improve hygiene and prevent viral infections. Avoiding irritants like cigarette smoke, taking it easy during the game even when your team is winning so that you don’t strain your muscle are great ideas that help prevent sore throat.
 
If you experience difficulty with breathing and/or swallowing or you see blood as in your case, please see your doctor asap.
