Daily Health Tips: I Gave A Soft Drink (Soda) To My 4-Month Old Baby!

Q: Dr. good afternoon, please, help me I gave coke to my 4 Months old daughter yesterday, now she just went to toilet now and I discover she has jedi-jedi. Please what drugs should I buy for her?

A: Okay, I know that different people wean their babies with different foods but there are a couple of issues here: first at 4 months, your baby should still be on exclusive breast feeding until she’s 6 months old. Then, when weaning starts, this should ideally be started with cereals. Remember that you should be modeling the right nutrition behavior for your baby now. If you introduce her to a diet of refined sugars now, it may be difficult to get her off them later. Recall that diets like this open her up to obesity and other lifestyle diseases. Caffeine found in some of these drinks can cause headaches, tummy upset, increased heart rate and increased blood pressure, amongst other effects. Having said that, what’s done is done. How to move beyond this?

If your baby is still taking breast milk, please go back to this…hoping that you will still make enough breast milk, if you have already introduced other foods to baby’s diet. As for jedi-jedi, what exactly do you mean as the term is used interchangeably to mean haemorrhoids (pile) and/or dysentery (poop with blood or mucous in it)? If you mean any of this and your baby still has this, please take her to see her paediatrician as soon as possible.

Daily Health Tips: Travel Restrictions During Pregnancy

Q: Good day ma’am. Please, I want to find out if it’s risky to travel by road. I just discovered that I am pregnant and I’ll be travelling from Osun State to Cross River State.
A: Travelling during pregnancy whether by road, rail or air is generally safe for women with healthy, uncomplicated pregnancies, especially in the 2nd trimester, when you would have stopped the puking 😀 associated with first trimester and before you start to get very tired and your tummy begins to announce your presence as you get into the third trimester 😉 The obstetrician also has to be a part of this decision making process…he may very well advice against travel for reasons like pregnancy being high risk etc.
Here are some general tips if you plan to travel:
• Be sure to have travel insurance or health insurance, if this applies. You just don’t know if you’ll need medical help. If you’ve got travel insurance, be sure it covers you for where you are. If you’ve got health insurance, be sure it covers you in emergencies at different locations within the same country.
• Get people/porters to help you carry your stuff, especially if heavy.
• Be sure to drink lots of fluids, especially water. Electrolyte imbalance which happens when one is dehydrated is not ideal for either baby or mother.
• Take your antenatal drugs along
• Store your doctor’s phone number…just in case.
• Wear loose comfortable clothing….I’ve always wondered at those beautifully made up and gorgeous women who I see all dressed to the nines and wearing serious heels to boot when taking a flight (sometimes at night too!) So, this advice is not just for pregnant women, please dress comfortably for these trips 😀
If travelling by road:
• Be sure to get up and walk around as much as practicable if traveling in a big bus. Hold on to railings, if available, to avoid falls.
• Otherwise, try moving your ankle around from time to time and when the bus/vehicle stops for a bathroom break, be sure to come down and move around.
• If one has been diagnosed with blood clots, special compression stockings may be needed for the trip
• Seat belts should be used. I know a lot of women who absolutely hate the idea of this. But this exposes you and your baby to unnecessary risks. The lap belt should be under the tummy and the shoulder belt should be between the breasts and off the side of the tummy
If travelling by air:
• Check the airline’s restrictions on travel by pregnant women. Airlines would usually allow travel up till the 36th week of pregnancy
• Move around often or wiggle ankles to exercise.
• Aisle seats may be a great idea as there may be frequent bathroom breaks. You don’t want the guy sitting next to you to rue the day he booked that flight, right?! ;D
• Use your seat belt.
• And hold on to the backs of seats to prevent falls.
If travelling by sea/taking a cruise:
Be sure that you’ve taken necessary vaccines and if you suffer from sea sickness, it may be a good idea to give this a miss.

Daily Health Tips: I’m Pregnant And Have Been Using Chloramphenicol. Is That Safe?

Q: Good morning Dr,please I am 34weeks pregnant and discovered I feel pain in my left ear. I did not go to my Dr but I am using Chloramphenicol (due to itching and I used cotton buds but I think I injured myself) will it affect me or my baby,help am scared. Should i go to my DR?

A: Chloramphenicol is an antibiotic which is usually used to treat various infections in the body. It slows down the growth of bacteria by preventing them from producing the proteins that they need to survive and multiply. It can be used in ear and eye infections etc
34 weeks? Wow! Steady on…you’re almost there!

Pregnancy is a very delicate time…a new life is forming within and the mother has to sustain this life by making all the right decisions. This does not include popping pills or taking un-prescribed medications at will. A significant number of drugs are not advised for use in pregnant or lactating mothers. If they are to be used, the focus is always on ensuring the benefits far outweigh the potential risks. Chloramphenicol eardrops may not necessarily be absorbed in sufficient quantities as to cause any problems for the mum or baby, but its use in pregnancy is not advocated because the drug crosses the placenta.

So, what to do? Discontinue this drug and whether pregnant or not, do not self medicate. That’s what doctors are there for! 😀 Go to them for advice and/or treatment.

Daily Health Tips: Why Do I Always Feel Sooo Tired?

Q: Dr. each time l return from work l look so unreasonably tired. What can I do?

A: I’m sure most people can identify with this. I’m assuming that beyond looking tired, you do feel tired too. That tiredness that appears to be a permanent fixture especially in a city like Lagos, Nigeria as we all attempt to out-run each other in the rat race 😀 In most instances where no specific medical ailment can be found, stress is the biggest culprit…what with multiple bills to pay, traffic, insurgency and terrorism etc

Other causes include:
• Psychological issues: Anxiety and depression
• Physical causes: Being under or overweight
• Lifestyle causes: Excessive alcohol abuse etc

To prevent this,
• Have a regular sleep routine and be sure that you have good sleep habits (no TVs in room, no work before bed time etc) and catch sufficient amounts of sleep daily.

• Drink sufficient amounts of water and eat healthy. 5 small meals comprising of three major meals and 2 snacks are useful, ensuring that your blood sugar level is maintained at an optimal state. Be sure to ensure that your meals have more fruits/veggies, proteins and complex carbs like local rice, oat meal, beans etc

• Reduce your consumption of alcohol or stop all together 🙂

Daily Health Tips: Urinary Tract Infections In Men

Q: Hello Dr Ketch. Please could you furnish me with the cure for a UTI? And what it does to a man if left untreated? Thanks.

A: There are bacteria that live within the intestines of human beings. They do not necessarily cause problems there. When one, defaecates (poops), the bacteria may be found at the anal opening. If these bacteria are then mistakenly carried from the anal opening to the opening of the urethra (the tube that carries urine from the bladder outside) through improper clean up after defaecating (cleaning from back to front) or after sexual intercourse, a urinary tract infection can result.

From the description of how this can occur, it’s fairly obvious why this is very common in women (women’s anal openings are closer to the urethra and their urethrae are also shorter). In men, UTIs are not so common and would happen more in people who have an enlarged prostate, people who have a poor immune system and people who have kidney and bladder infections.

Symptoms include frequent urination, difficulty with urination, pain during urination, fever etc. Involvement if the kidney leads to loin pain in addition to worsening of feelings if illness.
The doctor will usually send a urine sample to the laboratory to check for bacteria present in the urine and what antibiotic(s) are effective against them. Be sure to take the antibiotics for the prescribed period of time, even if you feel better before you finish the course to prevent backtracking of infection into the kidneys. Drink a lot of water and take analgesics for pain, if needed.

In summary, your specific treatment depends on the organism that is found in your urine at the laboratory. Be sure not to prescribe antibiotics for yourself! 😀

Daily Health Tips: What Is Douching?

Q: Good morning Dr Ketch, I liked your page some weeks back and I must say it’s quite educational. May God continually bless you. Please Dr, I need you to throw more light for me on the word “douching” Thanks

A: Thanks to all of you who like this page and visit every day for health tips. I’m glad to have touched your lives in some way 😀
Douche is a French word which means to clean or soak. People who douche usually inject/spray a mixture of water and vinegar into their vagina. Other douches contain antiseptics and fragrance too. They believe that douching serves one or more of the following functions:

• Cleaning the vagina
• Cleaning out menstrual blood after a period
• Prevent pregnancy
• Prevent STIs
• Get rid of odour

Unfortunately, douching serves none of those purposes and indeed, may even be harmful.

To clean the vagina during or after a menstrual period, one does not really need anything other than water. Mild, unscented soap may also be used on the outside. Douching disturbs the natural balance between the good and bad bacteria which live in the vagina, causing other organisms like yeasts to over grow the vagina. So avoid this and other scented feminine hygiene products.

Douching does not prevent pregnancy as loads of sperm may have already swum up to the womb before douching is done, after sexual intercourse. In fact, for those that may not have reached the womb (uterus), the force of douching may very well help deposit the sperm firmly in the uterus 😀 Talk about a boomerang!

STIs cannot be prevented by douching. In fact, if anything, if there is a vaginal infection, douching can actually push it all the way up to the fallopian tubes, potentially. These may lead to pregnancy complications in baby, labor and indeed, preterm delivery.

The vagina has a natural musky smell and one doesn’t really need douching for this. If, however, one suspects that the smell is foul and the discharge, thick, white and cheesy or even yellowish/green, there is pain when urinating and during sexual intercourse, a visit to the doctor would be helpful in making a proper diagnosis.

In summary, leave douching well alone! 😀

Daily Health Tips: How Can I Speed Up My Mother’s Recovery After A Partial Stroke?

Q: Good morning Dr Ketch. Thank you for your constant advice. May God richly bless you and keep you in good health. Please I need your advice. My mummy had partial stroke four years ago and she is getting better but not as fast as I want her to. She still walks with the aid of a walking stick and the unaffected leg bends backwards each time she walks. Please is there any drug she can take to help speed up her recovery? My brother massages her after she takes her bath every morning and she does some exercises on her leg and arm every morning. Also, she’s naturally a slim person but ever since the sickness, she has lost so much weight and I’m really concerned. Please what type of food should she take? I need her to put on flesh because seeing her the way she is presently makes me sad. Thank you and God bless you.

A: Stroke as the name implies refers to an incident that happens suddenly…without warning. It usually occurs when the brain is deprived of some of its blood supply and consequently oxygen supply. This can happen when there is a blood clot in a vessel in the brain or due to a burst blood vessel leaking blood onto the substance of the brain or around the brain. This blood leakage can lead to undesired pressure on the brain.

There is really no such thing as a partial stroke. Oftentimes, people refer to a Transient Ischaemic Attack, as a partial stroke. This condition occurs when a blood vessel is partially blocked. This would usually be resolved within 15 minutes and is a sign that a major event (a stroke, proper) may not be far off. This is a warning that should be taken seriously. Given how long your mum’s symptoms have lasted, her condition is more likely to have been a stroke, proper.
It is important to know the signs of a stroke because if action is taken immediately, it can prevent permanent damage from being done. If the brain is deprived of its blood supply for about 4 minutes, irreversible changes begin to occur in the brain. If this deprivation continues beyond this time, permanent changes would have occurred that may affect different facets of the individual’s functions.

The signs of stroke to look out for are known by the acronym FAST.
• F stands for drooping of a side of the face. Ask the patient to smile and you will observe that the smile looks lopsided.
• A stands for weakness of one side of the body. Ask patient to lift both arms and it will be seen that one side is being ‘dragged’ or lagging
• S stands for speech difficulty like slurring. Patient cannot enunciate words properly again
• T stands for time. Once these symptoms are notices, it’s time to call for help. In climes where there are emergency numbers like 911 or 999, please call these. Otherwise, efforts should be made to take the person involved to the hospital immediately.
Patients may also complain of sudden inability to move legs, feel confused, have blurred vision etc

There are risk factors for stroke and they include:
• Age: The older you are the more prone to stroke. People from age 55 years are more likely to have a stroke, though this can also happen in children eg those born with heart defects and those with Sickle Cell Anaemia etc
• Race: Africans and Asians are more at risk
• Sex: Females are more at risk…as if we don’t have enough problems 😀
• Family history of stroke: The presence of this increases risk
• Previous history of a stroke.

Other risk factors:
• Hypertension
• Diabetes Mellitus
• Overweight
• Sedentary lifestyles
• Not eating healthy
• Smoking
• Drinking Alcohol

The rate at which one recovers from stroke and indeed the degree of symptoms experienced, depends on the area of the brain affected, how serious the damage to the brain is etc and so recovery will be different for different people.

Patience is key. Speech therapists may be needed to help the person re-learn the art of speaking. Physiotherapists will help improve motion on the affected side and indeed, mobility. Other therapists may help with eating…if swallowing is a problem, relearning the art of reading and reasoning etc

Usually, people who visit find it difficult to understand their speech during recovery and this can put even more pressure on the patients. Speak slowly and listen carefully so you can understand what they are saying.

Perhaps, swallowing is an issue in the case mentioned above. Apart from that, if Diabetes is an issue, she needs to be on a strict diet and that will determine what she can or cannot eat. This will ultimately affect weight gain or loss. If this is a big concern, please discuss with her doctor. She may need to walk with a stick as she re-learns the art of walking. Recovery may be long and tedious but you’ve got to keep encouraging her.

Daily Health Tips: How Long Should I Let My Baby Sleep Without Waking For A Feed?

Q: Good evening, doctor. Please, I want to ask you how long I should allow my baby to sleep? He’s only 7 weeks old and he sleeps much especially during the day. Today, for instance, he has been sleeping from past 9 a.m till now and it’s almost 5p.m. I’m afraid he might be hungry though he wakes when he is hungry. But please doctor, is this ok for him? If not, for how many hours should I allow him to sleep before I wake him? God bless u doctor!
A: In the first few days and weeks of your baby’s birth, he’s likely to spend a lot of time sleeping. Indeed, he may be sleeping up to 16 hours per day…but really not at a stretch, usually. Ordinarily, he should be breast fed on demand and that means everything he asks for a feed. The problem is that your baby may just forget this all important function, ‘feeding’ 😀 and sleep for a looooong time! At this stage, you should never let your baby sleep for more than 4 hours at a stretch without waking him up for a feed. When counting the hours between feeds, remember that you are counting from when your baby started feeding and not when he stopped feeding. For instance, if your baby started feeding by 9am and finished at 9.30am; if he sleeps off, he’s due for another feed by 1pm and not 1.30pm. He should regularly be having about 7 to 9 feeds per day now. And just to be sure that he’s getting enough to eat, he should be having at least 8 diaper changes per day.
As your baby grows and becomes more aware of his environment, his sleeping and feeding needs will change to accommodate his new stage. Where in doubt, always have a chat with his paediatrician. The times you go for your baby’s vaccinations should be good times to check in with the doctor.

Daily Health Tips: Does Celibacy In A Man Cause Abdominal Cramps?

Q: Good evening ma’am. Please, if a guy is having abdominal pains or cramp…some said it’s lack of sex, as in sperm accumulation. What could be the cause and what is the remedy? Thank you
A: There are various causes of abdominal pain in men which include constipation, indigestion, lactose intolerance, ulcers etc but I can safely announce that staying off sex is not one of the causes 😀 You are more likely to get an STI by indulging in sex with unfaithful and/or unfaithful partners and some of these STIs may manifest with abdominal ache.
So, see your doctor to find out the cause of the abdominal pain 😉
