Q: Hi Doctor. What are the symptoms of stroke? And what causes stroke?

A: Stroke as the name implies refers to an incident that happens suddenly…without warning. It usually occurs when the brain is deprived of some of its blood supply and consequently oxygen supply. This can happen when there is a blood clot in a vessel in the brain or due to a burst blood vessel leaking blood onto the substance of the brain or around the brain. This blood leakage can lead to undesired pressure on the brain.

Oftentimes, people refer to a Transient Ischaemic Attack (TIA), as a partial stroke. This condition occurs when a blood vessel is partially blocked. This would usually be resolved within 15 minutes and is a sign that a major event (a stroke, proper) may not be far off. This is a warning that should be taken seriously.
It is important to know the signs of a stroke because if action is taken immediately, it can prevent permanent damage from being done. If the brain is deprived of its blood supply for about 4 minutes, irreversible changes begin to occur in the brain. If this deprivation continues beyond this time, permanent changes would have occurred that may affect different facets of the individual’s functions.

The signs of stroke to look out for are known by the acronym FAST.
• F stands for drooping of a side of the face. Ask the patient to smile and you will observe that the smile looks lopsided.
• A stands for weakness of one side of the body. Ask patient to lift both arms and it will be seen that one side is being ‘dragged’ or lagging
• S stands for speech difficulty like slurring. Patient cannot enunciate words properly again
• T stands for time. Once these symptoms are noticed, it’s time to call for help. In climes where there are emergency numbers like 911 or 999, please call these. Otherwise, efforts should be made to take the person involved to the hospital immediately.
Patients may also complain of sudden inability to move legs, feel confused, have blurred vision etc

There are risk factors for stroke and they include:
• Age: The older you are the more prone to stroke. People from age 55 years are more likely to have a stroke, though this can also happen in children eg those born with heart defects and those with Sickle Cell Anaemia etc
• Race: Africans and Asians are more at risk
• Sex: Females are more at risk…as if we don’t have enough problems
• Family history of stroke: The presence of this increases risk
• Previous history of a stroke.

Other risk factors:
• Hypertension
• Diabetes Mellitus
• Overweight
• Sedentary lifestyles
• Not eating healthy
• Smoking
• Drinking Alcohol

The rate at which one recovers from stroke and indeed the degree of symptoms experienced, depends on the area of the brain affected, how serious the damage to the brain is etc and so recovery will be different for different people.

Patience is key. Speech therapists may be needed to help the person re-learn the art of speaking. Physiotherapists will help improve motion on the affected side and indeed, mobility. Other therapists may help with eating…if swallowing is a problem, relearning the art of reading and reasoning etc

Usually, people who visit find it difficult to understand their speech during recovery and this can put even more pressure on the patients. Speak slowly and listen carefully so you can understand what they are saying.

Generally eating healthy with lots of fruits and vegetables, cutting out the white carbs (pastries etc), exercising (at least 150 minutes weekly), ensuring you are not overweight and quitting smoking help with reducing bad cholesterol and also preventing strokes.

 

She may need to walk with a stick as she re-learns the art of walking. Recovery may be long and tedious but she’s got to keep at it.

A: Strabismus (also called crossed eyes or squint) is a condition in which the eyes are not aligned when looking at an object. In this condition, as one eye looks forward, the other eye may turn upwards or downwards or look inwards. Strabismus could be congenital (where the child is born with it) or could develop before the age of 5 years. In these instances, it could happen as the child tries to deal with long or short-sightedness. It can also be seen for the first time in adults and would usually be due to an underlying disease condition like stroke.

Strabismus can lead to double vision in adults or lazy eye in children as they try to suppress vision from the affected eye. The head may be tilted in abnormal positions as they try to focus.

Personal hygiene which involves taking care of ourselves with activities like taking a bath, brushing our teeth, washing hands etc

Hygiene at home which involves keeping our homes clean with activities like cleaning the bed rooms, bathrooms, toilets and areas we hardly think of which harbor a lot of germs like door knobs, light switches etc

Food hygiene which involves preventing food contamination and preparation of foods in a healthy and hygienic manner

Pet hygiene which involves ensuring we properly clean up whenever we have been in contact with animals, pets or otherwise.

Now to hygiene-related questions:

Q: Can micro-organisms live in cold places eg food in a refrigerator?

A: Absolutely, they can. Cold temperature only slows down the growth of micro-organisms but it doesn’t completely stop them. So, the better the condition of the food stored in the refrigerator, the longer it will keep and the worse the condition, the shorter the period it will keep. Bear in mind that these foods will not keep indefinitely.

Q: In a situation where husband and wife share a room, can it cause cross-infection?

A: Husband and wife should definitely share a room because this promotes bonding and emotional connection. Can cross infections happen as a result of this? Well, if one person were not observing hygiene rules and, for instance, developed ringworm, this infection could very easily be passed on by sharing bed linen, towels etc. So, the solution is to ensure that everyone obeys the simple rules of hygiene.

Q: Please advice on dry gutters in or around the house. Also, dirty water on the floor in the compound.

A: Gutters (drainage gutters) around the house should always be scrubbed and cleaned. If not, they become a source of bad smells and pooling of stagnant water which encourage the growth of mosquitoes etc. Dirty water on the floor can lead to accidental falls and if the water is sufficient to become stagnant, again it becomes a breeding site for mosquitoes and other germs. Other probable breeding places for mosquitoes in the house are buckets of water, bathrooms with blocked drains, flower pots with more water than was necessary used for watering etc

Q: Are females to wash their private parts with soap? Some people say it’s not good to wash with soap.

A: Generally, washing the vagina requires just water and your hands. That is more than sufficient to get the job done. If you wish, however, you may use unscented or lightly scented soap.

Q: How can one maintain a younger look…that is not looking as old as your age?

A: Well, cosmetologists will have a million and one things they can suggest you use. I will go from the medical angle. First, your mental and spiritual health…be at peace with all men and just keep it positive. There are bound to be people who wake up everyone making it their job to rile you…put it in a bubble and blow it away 😀
Then, your diet. Eat more fruits and veggies, Substitute empty carbs with complex carbs, drink a lot of water. This should help keep you looking very young 😀

Q: Ma’am, I sweat a lot even on my palms and under my feet, so when I put on shoes, my feet smell a lot. Please, ma’am how can I reduce sweating on my feet and palms?

Excessive sweating may happen in very hot weather, obesity, stress or medical conditions like thyroid problems or Diabetes Mellitus. Your doctor needs o check out if any of this is the problem. However, in the meantime, ensure you take a bath at least twice a day and use deodorants/anti-perspirants to ensure there’s no associated body odour. Then if the weather is hot, dress light…wear cotton fabrics that are not dark and not tight fitting. And try and substitute sandals for shoes. Oh and by the way, for guys, please change your socks and boxers daily to prevent an odour.

#HLWDK Daily Health Tips: Treatment For Haemorrhoids In Pregnancy

Q: Good morning Doc, please can a pregnant mother insert anushole?

 

A: Piles, known as haemorrhoids in medical lingo are swollen veins in the anal canal. They can be internal, external or both internal and external can co-exist. Internal haemorrhoids occur when veins swell within the rectum and external haemorrhoids are found under the skin around the anus.

 

These haemorrhoids develop when there is undue pressure in the pelvic and anal area as can occur when people strain to pass faeces (if they have diarrhoea or are constipated), in pregnancy (especially the last 6 months), in obese people, people who eat diets low in fibre and people who practice anal intercourse. When women strain in labour, haemorrhoids can also be made worse.

 

It is important to consult your doctor/obstetrician before using any medication in pregnancy including this.

 

In the meantime, limit the discomfort you have by keeping the anal area clean, using wet wipes (non-perfumed) or washing with unscented soap to clean up after using the loo, sitting on a bowl of plain warm water for about 15 minutes about twice or three times per day, applying cold compress and not standing or sitting for too long. Some believe sprinkling some powder there helps…it doesn’t! Avoid it J

To prevent this, ensure that your diet has sufficient amounts of fruits and vegetables. Remember that if you take a regular flat plate, half of this should be filled with fruits and vegetables, half of the plate with complex carbohydrates and the last half with proteins.

Limit the time you spend sitting on the toilet seat (read your newspapers elsewhere :D), use the toilet as soon as you feel pressed (that way the fluid in the stool is not absorbed making the stool hard) and then don’t strain when you do go. Drink sufficient quantities of water daily, exercise (to keep everything moving along nicely :D). Use pain relief only under the supervision of your doctor.

#HLWDK Daily Health Tips: Bad Breath Issues

Q: Good morning Dr It’s an issue that have been given me sleepless night. Bad breath or mouth odor has been dealing with me for several years now, I have been to the dentist but no good response, I have used every thing possible but to no avail. I do change toothbrush every 3 months, using xxxx toothpaste immediately after brushing the odors will come up again. Please ma’am help me.

 

A: Thanks for writing in. I repeat below a post I had made earlier on bad breath in children and adults.

 

Bad breath at any age is traumatic but in a young child, it often leaves the parents wondering what they are doing wrong

Improper brushing of teeth is always an issue. People argue that they brush their teeth and their children’s teeth properly, but this may not be completely true. Do you brush your teeth for at least 2 minutes, paying attention to every section? Do you brush your tongue? Do you floss? Remember that food particles between teeth can interact with bacteria that live normally in the mouth and lead to stinking mouths…same for food debris at the gum line and tonsils. And then, when a baby has a congested nose and breaths with his/her mouth, the dry conditions enable the bacteria to thrive.

Tonsilitis, finger sucking by babies, tooth cavities and buildup of tartar on teeth can all cause bad breath.

 

What can you do as a parent? Get involved. Help your baby with brushing the teeth to teach him the right technique. (When you say, he brushes twice a day, I hope this doesn’t mean he’s doing it without supervision?) Brushing times can become fun bonding times for mummy and her son or for daddy and son. Sing nice fun songs and brush your own teeth at the same time too

 

Remember that oral care (care of the mouth) should start even before the first tooth erupts. Regularly wipe your baby’s mouth and gum with a wet flannel and as soon as the first tooth/teeth erupt, start brushing. Use tooth paste that does not contain fluoride until your baby can spit. What tooth paste does not contain fluoride? Ask at your regular supermarket or simply pick up the packs of the different tooth pastes and look for one that does not contain fluoride

 

J Use a tiny amount of paste at this initial time (about the size of a grain of rice), then graduate to a pea sized amount and tooth paste that contains fluoride, when the child can spit out.

 

Remember the rule of twos: Take your baby to see the dentist twice a year, help him and teach him to brush his teeth at least twice a day (three if you can manage it, after every meal) and be sure to ensure that he spends at least 2 minutes brushing his teeth. A simple way of doing this is mentally dividing the mouth/teeth into four quadrants/sections. Then spend at least 30 seconds on each side, making sure you brush every surface of the teeth.

 

Then be sure not to forget the tongue. Pay attention to every side and use a soft bristled brush to do a good job. There are tongue scrapers which can be purchased alone or tooth brushes which come with bristles at the back for brushing the tongue.

 

Stay off alcoholic mouth washes for your baby as the alcohol just worsens the dryness in the mouth, And remember that mouth washes are just temporary fixes. They only mask the bad breath and not treat it.

 

Encourage your baby to drink water regularly, especially if they have dry mouths and they’ve got to scrub their hands regularly too, if they suck their fingers/thumb.

 

These should be sufficient to stop the bad breath. If they don’t however, please see your doctor to rule out other causes of bad breath which cavities in the teeth etc

 

And for adults…..

Bad breath, also called halitosis, may be as a result of poor hygiene habits, eating strong smelling foods or could be a sign of other medical conditions.

If adequate dental hygiene is not maintained, food particles can remain in our mouths, encouraging bacterial growth bacteria in our mouths can break down food particles and raise a serious stink. Proper dental hygiene habits includes:

Brushing your teeth twice a day (this sounds like a ‘no-brainer’ but the right technique is important. Mentally divide your mouth into four quadrants and spend at least 30 seconds on each quadrant. This way, you spend a minimum of 2 minutes brushing)

Brushing your tongue (if you finish brushing and you can still see all that whitish residue on your tongue, you haven’t done a good job. Some of the smell comes from there. Some tooth brushes come with a tongue scraper at the back of the brush…use that. If not, buy one or just use your brush)

Flossing your teeth everyday. Ask for this in your local super markets. This gets rid of the food particles between your teeth where your tooth brush can’t reach.

 

Using a mouthwash can also help the prevention of mouth odour.

Eating strong smelling foods like onions and garlic could also cause mouth odour, which may not be taken care of by brushing. This is because these ‘foods’ are absorbed into the blood stream and excreted through the lungs. So, the smell will persist until excretion is complete. The small can be masked with mouthwash etc

 

Disease conditions that can lead to bad breath include mouth infections, Diabetes Mellitus, sinus infection etc.

 

Other conditions like use of tobacco (chewing or smoking), some drugs and dry mouth (which is the reason for morning breath :D) are also culprits.

Prevention is focused on the tips given above and ensuring that you visit your dentist at least twice a year, brushing at least twice a day (if not after every meal) with a fluoride tooth paste (remember that fluoride is not recommended for children less than 2-3 years and even then, only a pea size is recommended), change your tooth brush once every three months and drink lots of water.

 

Let’s keep the pearls shining and the breath smelling really fresh.

By the way, would you tell a friend, if he/she had bad breath?

I asked that question during one of my shows and most people didn’t want to be the one that causes offence by telling someone that they have bad breath. However, most people also wanted to be told if they had bad breath. So, who will bell the cat? 😀

People who go for In Vitro Fertilization may have the option of selecting their baby’s sex, but even this is not offered indiscriminately.
Spacing your children allows your wife to rest in between pregnancies and recover her strength. It does not increase your chances of having any particular sex, neither does having them in rapid succession. It’s just not up to you! That’s just the point 😀
I know that traditionally, we like to think that our family lines can only be propagated through the male species, but remember Obama and Clinton, two great men who have only girls as children. I don’t think the world will forget them in a hurry! Your daughters will bring you absolute joy and happiness and perhaps cater for you in ways that you would not have even expected. Both genders are important and we need to accept both as gifts from God.

#HLWDK Daily Health Tips: How Do I Store Breast Milk?

Q: Good afternoon doctor. Please as a working mother how do I store breast milk for my baby because I want to do exclusive and is there side effect for breast pump? Thank you

A: Expressed breast milk has to be handled in a very hygienic way. It is important that it does not form the means by which germs find their way into your baby’s body so utmost care must be taken. What should you do? I reproduce a post I had made on this a while back; To express milk for your baby, first, you have to be committed to be hygienic because you don’t want to introduce germs into your baby, do you? Always scrub your hands with soap and water whether you are using the breast pump or your hands.

Having expressed the milk, when stored at room temperature, be sure to use up the milk within 6 hours. When stored in the fridge,

• Ensure that the temperature in the refrigerator is below 5 degrees centigrade.

• Do not store on the door of the fridge. Please store on the shelf towards the back of the refrigerator where it’s cooler

• At this temperature, you can store milk for up to 5 days.

When stored in the ice compartment of the refrigerator, you can store milk up to 2 weeks

When stored in the deep freezer, you can store milk up to 6 months. To thaw frozen milk, please put it in the fridge.

When it’s thawed, you may feed your baby with it but if your baby prefers it warm, you can run some warm water over the bottle until it’s just right. Do not microwave it as this may create some hot spots in the bottle which may cause scalding of your baby. Once thawed, please don’t re-freeze the milk as this might encourage the growth of micro-organisms.

Make sure that the container used for storing milk is sterilized to prevent any potential introduction of germs into the milk. Use small containers that hold just about the amount that your baby finishes per feed. If you express a lot of milk, be sure to label the different containers and use the oldest pack first. Please stick with the duration of storage guideline we’ve given.

Now, it goes without saying that you should not store milk close to contaminants like meat, fish or even strong smelling foods like onions 😀

There are no side effects of breast pump, provided you are using it properly. Personally, I found that using my hands to ‘milk’ my breasts produced sufficient amounts of breast milk for my always-hungry-can’t-seem-to-get-enough-food-baby. You may want to try that.
