Daily Health Tips: Dripping Nose!
Q: Good Evening Dr. Please, I need your help on this. I have been married now for 7yrs and I dated my husband for 2 years. Please, the problem is running nose. His nose never dries. In fact, it’s worse at night. Please, what can he use? Thanks
A: Well, it may be due to allergy known as allergic rhinitis (pronounced Rine-Ai-Tis). This condition, also known as hay fever, occurs when your immune system over-reacts to certain substances in the air (allergens), marking them out as intruders.
The resulting symptoms are due to some chemicals released in the body like histamine. These substances cause sneezing especially when you wake up in the morning, runny nose, watery itchy eyes and itchy ears, nose and throat, which could occur seasonally or be present all year round. In fact, the symptoms written above are as good as a text book description of this condition 😀 Oftentimes, in people with this condition, there is a positive history of a family member with asthma or other allergies.
The first step towards dealing with this is finding out what the triggers (allergens) are. It could be dust mites, wood dust, smoke, pollen, grasses, cockroaches etc. Your doctor will ask helpful questions to help you identify the culprit. He/she, in the meantime, will prescribe some drugs, the commonest of which is an anti-histamine to help control your symptoms. Other treatment options include taking allergy shots and going for a nasal irrigation (essentially rinsing out the mucous from your nose).
Prevention is focused on identifying and reducing contact with allergens. This would include eradicating cockroaches from your house, using a mask when working outdoors, closing doors and windows when there are lots of pollen in the environment, not hanging laundry outside to prevent pollen sticking to them and causing problems later etc. Reducing dust mites by using oiled duster or damp cloth to clean, using allergen proof covers on mattresses etc will be helpful.
So, get a doctor to prescribe drugs to tide you over (you should be able to pick up the prescription over the counter thereafter) and work with you to identify allergens and then you have to avoid them.
Other causes of runny or stuffed nose include:
Common cold. We are all familiar with this common cause of nasal congestion 😀 This causes blockage of the sinus which results in nasal congestion.
A nasal septum (the wall between the nostrils) that is crooked or bent to one side may cause nasal congestion. This ‘deformity’ may be from birth, due to injury or occur as a process of growth.
Excessive use of nasal decongestants can also lead to this as after the decongestant is stopped, there is rebound nasal congestion. And so, one ends up with the same situation they were trying to solve from the beginning! 😀 So, if you must, do not use your nasal decongestants for more than three days
Sinusitis is an inflammation of the sinuses (air cavities) that line the nose. When these air cavities (which are supposed to be filled by air) become filled with fluid as a result of blockage, bacteria, fungi and viruses can thrive in this environment and cause infection. Pus-like nasal discharge, facial pain and persistence of symptoms for more than a week, maybe suggestive of this.
Other conditions include:
• Cold weather
Foreign body in the nose
Presence of tumour or polyp (a small growth with a stalk protruding from a surface)
Medications for high blood pressure
Spicy foods (remember how you start sniffling once you start on a hot dish?)
Stress etc
There’s a pretty long list of causes, but your doctor will attempt to narrow down to what the probable cause for you is.
In the meantime,
Ø try drinking a lot of water
Ø Use saline sprays instead of the nasal decongestants, if you’ve been indulging in this 😉
Ø Use a humidifier which loosens the mucous. Turning on the hot water shower and inhaling the steam is another great idea.
Ø Avoid irritants of the nose like smoke
All these measures will help reduce the nasal congestion.

A: Thanks for writing in.
When you have a brand new baby, one of the investments (which thankfully does not require a lot of money) you should make is to buy a mosquito net. This protects your very vulnerable baby and prevents malaria which can be quite severe in this age group.
Children become especially vulnerable to malaria at about 3 months of age when the immunity they received from their mothers start to reduce.
First tip is, do not treat your baby at home. Take your baby to the hospital. The doctor needs to confirm that what your baby has is indeed malaria. This involves running a test to check for the presence of malaria parasites.
The treatment of choice is still Artemisinin-based Combination Therapy (ACT). Be guided by your doctor for dosage and duration of treatment.

‘Pregnant Daddy’
Pregnancies affect daddies too! First, this amazing woman they love suddenly becomes a bundle of emotions. He’s not sure from day to day, what is appropriate. Today, he wasn’t affectionate enough; tomorrow, he’s too affectionate in front of people? And the bursting into tears at the most improbable times? What’s going on? Is this the picture of marriage he has to deal with for the rest of his life from now? ‘And then, there’s the constant chatter about the baby!
Honestly, I get it. I’m excited too. This is our first baby. A symbol of our love for the whole world to see. I truly can’t wait to also see whether he/she will look like me or her (honestly, I don’t even really care!), whether he/she will share those character traits from my family that have been strong through a lot of generations etc. But honestly, I don’t want to spend every waking moment talking about that nor picking out clothes, colours etc. I sincerely want to be there for my wifey but this is just too much. Am I being selfish?’
No, you’re not! It’s a normal reaction which you’re entitled to, but really have to deal with quickly😀 That didn’t sound very sympathetic, right? Some of the issues on the minds of fathers are worries over an ‘impending’ starvation of sexual intimacy, increase in financial responsibilities etc. These are enough to stress the potential Daddy out and lead to the release of the stress hormone, Cortisol, which reduces libido. But, pregnancy does not mean starvation of any kind😀

Daily Health Tips: Are Painful Nipples A Sign Pregnancy?
Q: Please ma’am, I am having pain in my nipples since three days now. Could it be a sign of pregnancy because I had unprotected sex with my man or should it be the sign of my menstrual cycle? I am expecting it on the first of next month.
A: There are many different causes of nipple pain and we will explore some here.
• Given the fact that sexual intercourse happened just before the complaint of nipple pain, it may very well be that this led to the pain. Vigorous rubbing of the nipples can lead to pain. This would usually heal without incident and applying a moisturizer will help the process.
• Friction: In this case, not wearing a bra or wearing a poorly fitted bra during exercise routines may lead to rubbing of the nipples against the fabric and consequent soreness/pain. Wearing a proper bra could help. Other products that cover the nipple are helpful. You could improvise by using cut-up band aid/plaster over your nipples before going for a run or exercising.
• Pregnancy and breastfeeding can also lead to this condition. So, you could be right. In pregnancy, the pregnancy hormones lead to the nipples being extra sensitive and during breastfeeding, the baby may not latch on well to the nipple, there could be blocked ducts or mastitis or breast abscess etc leading to pain. During pregnancy, be sure to choose bras made with fabric that can ‘breath’ like cotton. And during breastfeeding, be sure to get help with making sure you know how to put baby to the breast to prevent sore nipples etc.
Sensitivity to the weather (hot or cold), certain fabrics, detergents or even creams/lotions may lead to nipple pain. If this is caused by specific detergents or creams, you may have to change them.
• Hormonal changes as can occur close to a woman’s menstrual period. This pain will usually disappear after a few days.
• Cancer (Paget’s disease of the breast) can also be a cause of nipple pain.
So, where in doubt…especially if you have tried to rule out the causes above, please see your doctor.

Daily Health Tips: Thyroid Problems
Q: Good morning doctor, please I want to know the symptoms of thyroid and how it can be treated .
A: Hyperthyroidism is a condition that occurs when your body produces too much thyroid hormones. These hormones are usually produced by a butterfly shaped organ located at the front of your neck, called the thyroid gland. These thyroid hormones produced typically control the rate at which the body converts food t energy (metabolic rate) and also controls a whole lot of other cells and tissues like your bones (the hormones determine how your body incorporates calcium into your bones), the rate of your heart beats, your muscle contraction, cholesterol levels etc
Most cases of hyperthyroidism is caused by Graves’ Disease in which the body no longer recognizes the thyroid gland as part of it and produces antibodies to fight it. In response, the thyroid gland produces excessive amounts of thyroid hormones.
As in the case above, there may not be any family history though it is known to run in families.
Thus in hyperthyroidism, you may have no symptoms (leading to a chance discovery) or symptoms would include fast heart beats/palpitations, swelling of the thyroid gland which is seen as a neck swelling (goiter).The higher than normal metabolic rate would lead to symptoms like increased appetite, weight loss, sweating and increased sensitivity to heat, restlessness, tremors, tiredness, difficulty sleeping etc. In Graves’ Disease, there are also eye symptoms which include protrusion of the eyeballs, red and swollen eyelids, watering of the eyes and double vision.
The doctor will examine the person with symptoms and run blood tests to check for the level of thyroid hormones. Treatment could be with medications or through surgery or a combination of both.
The drug you have been prescribed has the active ingredient carbimazole which stops the thyroid gland from making new thyroid hormones but does not affect the hormones already produced. As a result, it may take a while to notice an improvement in symptoms. The use of this drug may be for about 12-18 months in the first instance. This may be sufficient to deal with the situation. In some instances though, this may recur needing more treatment, probably with a different drug.
You will need blood tests regularly though to monitor the level of your thyroid hormones.
Depending on your degree of weight loss, you may need to have a dietician walk you through your diet paces and ensure that you are getting enough calcium and vitamin D. Wearing sunglasses, using eye drops that moisten the eye (artificial tears) and ensuring that the head is elevated when sleeping or lying down will help reduce the symptoms of Graves’ Disease. If you smoke, stop…this may help prevent developing Graves’ Disease or prevent the eye problems associated with it, if you already have the disease.
Treatment for hyperthyroidism is usually effective and so you should be able to live a normal life. You’ve got to focus on the positives and take on each day as it comes. You may not have to take drugs all your life but then again, depending on the outcome of your initial therapy, you may have to. This may sound daunting but really shouldn’t be. Lots of people take ‘self-imposed’ vitamin supplements every day
Just have a positive attitude about it and learn to look at this cup as half full rather than half empty. Keep your chin up and keep me posted.
Conversely, in hypothyroidism, the body does not produce enough of the thyroid hormones due to an underactive thyroid gland. They symptoms include tiredness, weight gain, constipation, increased sensitivity to cold, depression etc
Causes of hypothyroidism include autoimmune diseases (when the body turns on itself, producing antibodies that attack and destroy the thyroid gland), inadequate iodine in the diet, radiation therapy to the neck, thyroid surgery (in which some or all of the thyroid is removed) etc. Other factors associated with this condition include pregnancy, certain drugs etc Risk factors for hypothyroidism include white race, growing older, having prematurely graying hair etc.
The major issue causing the symptoms is the lack of thyroid hormones and so treatment is targeted at that. Synthetic thyroid hormones will be prescribed to help combat these. Effects on fatigue may be observed as early as two weeks after treatment is started.
So, whatever your thyroid problem is, please see your doctor for a proper examination and he/she will make a diagnosis after which a course of action will be determined.

Daily Health Tips: Hormonal Migraine
Q: Hi Doctor, please I want know the reason why someone gets headache before and after menstruation and how can it be eradicated.
A: Headaches during menstrual periods are referred to as menstrual or hormonal migraines and are due to a drop in the level of estrogen, a hormone. For a headache to be classified as this, it would happen anytime between about 2 days before a menstrual period to about 3 days after the first day of menstrual period. Some of these women find relief during pregnancy as the level of estrogen rises.
To pre-empt this, you can take pain relievers that are also anti-inflammatory like ibuprofen at the start of the period or even about a few days before the start of an expected period until the bleeding stops. If you’re asthmatic or have peptic ulcer disease, this drug is not a safe choice for you.
Be sure to discuss this with your doctor and be guided by him/her.

Daily Health Tips: Can I Be Pregnant So Soon After A Miscarriage?
Q: Hello, kindly do not reveal my ID. I had a miscarriage last month. I was about 10 weeks ,but was told it stopped at 6 weeks. I went through D and C and bleeding. After seven days and there on I had unprotected sex with hubby. I was feeling strange so I decided to do a pregnancy test. It came out positive, could I be pregnant or not? It would usually take about four to six weeks after miscarriage for the hCG levels to return to normal. The Human Chorionic Gonadotrophin (hCG) is a hormone produced by the body when pregnancy occurs and this is sought for when pregnancy tests are conducted. So, this could be responsible for the positive pregnancy test you had.
Could it be pregnancy? It’s not farfetched. Ovulation could occur as early as 2 weeks after miscarriage and that means pregnancy becomes highly possible.
Other causes of rising hCG could be an ectopic pregnancy that co-existed with the miscarried pregnancy. A patient with ectopic pregnancy would usually come into hospital with complaints of light abdominal bleeding, abdominal pain or cramps, extreme dizziness from the bleeding. If the tube ruptures (bursts), patient could faint from profuse bleeding into the abdomen. This condition needs to be taken care of immediately. Rising hCG levels could also be due to two conditions known as choriocarcinoma (in which a cancer forms in the ‘placenta’ tissues) or a molar pregnancy (in which a fertilized egg turns into a mass of cysts instead of a normal pregnancy).
So, please see your doctor and have a long chat. He will advise on next steps.
