Daily Health Tips:How Do I Improve Sleep While Breastfeeding?

Q: What also do I need to improve sleeping very well at night since I have a baby that sucks all night and day?

A: For those new mummies with children, this may be more wishful thinking as our bundles of joy have minds of their own😀. We just wish sometimes that we could explain nicely to them that we….well, just need to sleep now; regardless of how much they want to chat or cry (!) especially in the middle of the night! Well, here are some helpful tips that ensure a good night’s rest for mum and new baby
•    During nighttime feedings, keep lights low.  Night lights stop you and baby from coming fully awake and so you can go back to sleep after the feed…effortlessly.
•    Commit to sleep and make it a priority in your life.  The advice frequently given to new mothers to get some sleep when the baby is asleep, may appear clichéd and ‘an old wives’ tale, but is useful.  Give yourself permission to let some things go and don’t feel the pressure to catch up on all your emails, text messages, house shores when he’s asleep.
•    Good sleep starts before you’re actually trying to fall sleep.  Ensure you start winding down your day early enough so you can get to sleep early at night. Ensure you’re not getting too wound up during the day and inhibiting your ability to sleep at night
•    Be intentional about sleep.  As a new mother, you’ve got so many things pulling at you: night time feeds, your other children, if you’ve got them, their homework, your husband etc. So ensure that your sleep equipment – pillows, mattress pad/topper and mattress – provide the right support and comfort. Get a good pillow and comfortable mattress…it’s a worthwhile expense
•    Be open to alternative remedies.  Create a personal spa at home: use aromatic oils at bath time and in your room. Lavendar has been found to be very helpful by some.

Daily Health Tips: Ovarian Cysts And Fertility

Q: Evening Dr Ketch. I hope you’re good. I.am only 18. I have been having abdominal pains and I was told that I have ovarian cyst. I am on medication but they said if it doesn’t work then I’ll have to undergo an operation. I am scared because they told me I have less chances of conceiving. Could this be true?? (I’ve never had an abortion before). Please help!

A: Thanks for writing in, dear.

Ovaries are small bean-shaped organs on either side of your womb. Every month, your ovaries develop sacs called follicles from where eggs are released monthly and female sex hormones produced. After the egg is released every month, the sac disintegrates. But sometimes, the egg is not released and the sac remains or the egg is released and the sac does not disintegrate. This follicle becomes the fluid-filled cyst sac. Usually this is not a problem as it could form during every menstrual cycle and usually resolves on its own. These cysts are called the functional cysts. However, these cysts could get very big and in some instances become twisted or they could rupture causing problems. This latter type is called pathological cyst.

 

Examples of pathological cysts occur in Polycystic Ovarian Syndrome and endometriosis. In the former, small cysts appear all over the surface of the ovary and is also associated with irregular menstruation and high hormonal levels. In endometriosis, the tissues of the uterus (womb) are found outside of the uterus. These two pathological cysts are associated with fertility problems. Functional cysts are hardly associated with fertility issues except they become extremely large.

 

Treatment depends on your particular situation. You could get away with no surgery and no pills, if your doctor orders serial scans to continually observe the size of the cysts and sees a reduction in size. Otherwise, the other treatment options are the use of birth control pills to prevent the development of more cysts in future menstrual cycles and surgery as the final option.

A: Aloe vera is great with many health benefits. But useful for weight loss in combination with lime? It sounds like one of those fad diets I have heard about. This is a variation of another question I have been asked before…

A: Hmmm! What did you have in mind? Taking the hot drink (alcoholic spirit) alone or in combination with the lime? Now, you have to forgive me, but I’m always amused at the fact that we think that there are short cuts to these stuff. Unfortunately, losing weight is pure hard work: sweating and grunting.

Alcohol’s health benefit is usually touted in the area of heart health where moderate alcohol consumption is thought to provide some benefits to the heart. Moderation is described as one drink of alcoholic beverage per day for women and also for men 65 years and older and two drinks for men younger than 65 years. However, even these moderate servings of alcohol are also associated with cancers and other disease conditions. Indeed, the beneficial effects of this moderate alcohol on the heart can be reproduced by paying attention to diet and exercising at least 150 minutes per week.
In addition, alcohol contains empty calories which will definitely mess up anyone’s weight loss plan and when people drink alcohol, they lose their inhibitions and may completely forgo their diet plan and eat all sorts

Daily Health Tips: Re-visiting Lumps In The Breast Of A Breastfeeding Mother

A couple of questions came up yesterday after my post on breastfeeding and I will be addressing one of them today.
 
Q: Thanks but what of if the mother has a lump in one breast? Should she still be giving the baby that particular breast?
 
A: Painful lumps in the breasts of breastfeeding mums may be blocked milk ducts. Usually your milk ducts block if you’re making milk faster than you’re expressing it and indeed you should feel some relief every time you breastfeed from that breast…so stopping breastfeeding may just be making a bad matter already worse 😀
 
What causes this milk duct to block? It could be that your baby is not latching on well to the breast and thus not sucking enough from that breast. It could be that you don’t feel well and are not feeding baby often enough, or you decided to wean baby suddenly. Other causes include tight nursing bra, sleeping on your tummy, a nursing pump that’s not powerful enough, a blow to the breast or it could just be that a feed was skipped.
 
 
Solutions? Nurse your baby some more, ever so often….especially from that particular breast 😀 Yes I said that. Baby sucks hardest then and hopefully that strong sucking action would dislodge the plug. Otherwise try a nursing pump or just use your hands to express the milk.
 
Placing warm towels on the affected area may also help the breast milk flow faster and thus dislodge the plug.
 
Rest as much as possible, eat healthy (loads of fruits and veggies) and use pain relief, like Paracetamol, if needed.
 
If this condition is not handled immediately, it could lead to an infection of that duct (mastitis) such that the skin over that area feels hot to touch, reddens, mum feels feverish, ill and aches all over. This infection is usually treated with a course of antibiotics, which will be prescribed by your doctor.
 
So, if you try all the suggestions above and don’t improve, please see your doctor to confirm if this is an infection or indeed something else.

Daily Health Tips: Should I Give My Baby Water While Exclusively Breastfeeding

Q: Good morning Doc, please I just had a baby and I want to do exclusively breastfeed my child. I don’t know whether I will be giving him water because they told me that water is natural.
 
A: Water is definitely natural but when exclusively breastfeeding, you don’t need it. Breastfeeding contains a significant amount of water and so if you breastfeed your baby adequately, he will get both ‘water’ and nutrients from your breast milk. For more on this, I reproduce a post I had made on this subject a while back…..
 
Over and over again, I have had breast feeding mothers tell me that they don’t make enough breast milk to feed their babies. Or they would say, their babies get hungry too often and so it is difficult to keep up with the crazy schedule of ‘feeding on demand’ that breast feeding requires of you.
 
Breast milk is very important to new born babies and really no child should be deprived on the many benefits of breast feeding exclusively. There are some mothers who, by reason of specific health conditions, cannot breast feed their babies. However, the greater majority of mothers are able to make enough breast milk to keep their babies satisfied.
 
Here are our top five tips for breast feeding properly:
o Patience: You’ve got to have loads of it. Sometimes, your baby feeds and then stops a bit just to gaze into your eyes. Give him/her the time needed to bond with you.
 
o There are two types of milk in the breast milk: the fore and the hind milk. You have to be sure that your baby gets both during each feeding session.
 
o Give the fore milk to quench thirst. The fore milk is mainly water and low in fats. If you observe, it’s bluish in colour. Its high water content is the reason why you don’t need to give your baby extra water. It’s also rich in protein, carbohydrates and vitamins.
 
o Be sure to give the hind milk. Now, if you finish giving just the fore milk and stop, the baby will be hungry thereafter as she has essentially only taken water. The milk that comes after the fore milk is thicker containing fats and has a whole lot of calories. The baby needs this to be very well nourished and to satisfy the hunger pangs. The longer a mother waits between meals, the more foremilk is produced and it takes your baby that much longer to get to the more filling hind milk.
 
o Let your baby feed on demand and each session should last until the breast ‘feels’ empty. I say ‘feel’ because the breast never really empties out After this, change to the other breast.
 
Remember that breast milk is the best and cheapest food for your baby. It also helps fortify your baby against diseases…so, now you have the right tools to get this done right.

Daily Health Tips: Should I Increase The Dosage Of My Slimming Medication?

Q: Good evening Doc! Please help me, I am overweight. Almost ten years older than my age. Recently I started combining some green teas and a slimming capsule which I take two at a time after breakfast. The capsule makes me lose appetite. This only worked for 3 days after which my longing for food returned again. Now the woman that sold the product to me advised that I increase it to 3 since 2 no longer works for me. I am scared. Will this have any effect on my health? I over eat and can’t help it. I am 25 and size 18. I am single and this has really affected my self-esteem. Please help me, ma’am. I need to trim down.

A: I get a lot of questions about slimming pills, green tea and those wonder drugs that make you shed weight in ‘seconds’ and instruct you never to eat of specific classes of foods. Let’s use green tea as an example. Green tea is great and studies have shown that it speeds up metabolism and so helps you break down all the food you’ve taken in, faster. However, weight gain is a function what you’ve taken in and what you’ve taken out. If you take in 3000 calories and you’re able to burn only 500 calories, then clearly, there is some excess that will get converted to fat et al. Remember that some of our ‘snack’ like some burgers pack as much as 500 to 600 calories in one serving….and that’s just one meal (or maybe even snack) with 3 more meals and perhaps another snack to account for. And so, if you are loading your system with more food than you need, then even the purest of green teas is going to have a hard time keeping up with your metabolism…there’s just too much work to do what with all that food! Remember that generally, women are entitled to 2000 calories intake in a day and men are entitled to 2500.

To practical solutions, there’s no magic wand tablet that makes it all go away. Hmmm, that’s not really true, so I’ll re – phrase. There are a number of take-a-tablet-daily-for-10-days-and-you’ll-lose-all-that-weight-in-the-same-period tablets and fad diets, but what these tablets and diets do is that they make you lose a lot a lot of water weight, which makes you feel like you’re losing weight and then once you go off the tabs or fad diet, you pile it all on again. So, the only solution I proffer is plain hard work 😀 focused on eating right and exercising. You are right to be scared and worried. Increasing the dose of any medication should only be done by a qualified medical personnel who understands the effect he is trying to achieve. This your friend does not sound like one! Truth is, a lot more work needs to still be done on weight loss supplements to confirm their safety profile. Please also note that the fact that a drug is labelled natural or herbal does not necessarily make it safe. A lot of these drugs have significant side effects which an increased dosage will only worsen. So, do not increase dose without discussing with your doctor…and indeed getting his take on your weight loss plan.

Some ask, can I lose weight without exercise? You can…if you watch your portions. However, exercise is useful for more than just weight loss. Exercise makes your heart strong and reduces your risk of other ailments.

And then some ask, I eat only once a day, how can I possibly add weight with that? If you eat once a day, chances are you snack on other things during the day that you do not consider as food…it could very well be cookies etc. But even if you don’t snack on anything, chances are that when you eat once, you binge and essentially make up for all the meals that you didn’t eat during the day….and so we’re back to square 1 😀 Remember that you’re actually allowed up to five small meals in a day. 3 main meals and 2 snacks.

I’m not and never have been a huge fan of fad diets. I think they place a lot of restrictions on you which make them impracticable and thus easily abandoned. So, for all of you who have written in asking for how to lose all that belly fat and perhaps if there’s a magic pill, here’s my answer. It is hard work…plain and simple.

 

First focus on portion control. Start by reducing the size of your plate. If you eat with a bowl, go for a plate. If you eat with a flat plate with a circumference that could hold the globe go for a smaller one etc. Then make sure that you focus on eating healthier foods. Half the plate should be filled with veggies, a quarter with protein and a quarter with carbs. Focus on complex carbs like brown rice, potatoes, yams, oats etc. Your palm is actually a good measure of how much you should eat and before you pelt me with rotten tomatoes for saying this, I’ve actually got very small palms so I’m suffering through this too

 

Then, exercise. This could be in the gym or through your regular activities. If you are a stay-at-home mum, dancing while doing your house chores, may not be what you have in mind when you look at a full house of chores, but it could start you on your fitness journey. And if you have a house help, don’t send her to do everything, get up and do some stuff yourself. Taking a walk in the evening when everyone is back, is also helpful. Going to the gym is a great way to start your day. In the office, move around some more…to the printer, to the bank, to the restaurant for lunch. Don’t call for everything.

 

And then, try and reduce your stress level…I can almost hear your ‘yeah, right! How do I do that?’ Well, stress is a normal part of life and in fact is needed to give you the regular push to get and go every morning. Find out your sources of stress and figure out solutions to them. For those you can’t find solutions for, let them go. No problem has ever been solved by stressing out over them. During stressful times, we are more likely to make wrong food choices. You may feel too lethargic to go anywhere and want to grab a quick food fix on your desk…where you’ve been sitting all day!

 

A couple more quick tips to help you limit your food intake, limit the episodes of hunger pangs and ensure you do eat right:

Take your time and chew every morsel. Beyond being in tune with etiquette 😀 it helps give your brain the time needed for it to receive the signal that the stomach is full. If you eat too fast, you would have stuffed yourself before you feel full. Remember that vegetables form soluble fibre which absorb water and thus give you a feeling of being full for a longer period of time.

You can have 5 meals per day: three major meals and then two snacks. Try to mentally confirm that you’re really hungry whenever you feel the urge to munch on something. Sometimes, it is thirst, boredom or some emotional stress that we are trying to deal with by eating.

Surround yourself with healthy snack and food options so that you are encouraged to do the right thing 😀

Remember that the path to healthy eating/living is long and winding but there’s always a pot of gold at the end of it. The short cuts always bring you back to where you started because they are not sustainable.

Start off today…the first couple of days are often the hardest but if you stick with it, you will feel so much better and be, indeed, healthier

Daily Health Tips: Anal Fissure

Q: Good morning Dr Ketch. Please, I have an anal fissure that has refused to go. I’ve changed my diet by taking foods high in fibre. Please, what do I do?

A: Constipation occurs when faeces passes too slowly down the digestive tract causing the stool to become hard and brittle. It leads to a situation where a person has less than 3 bowel movements per week. The classic symptoms are straining to pass stool, having the feeling of incomplete emptying after a poo and sometimes having to press down on your abdomen to help emptying. Sometimes, constipated people also insert a finger into their rectum to help the process along. Constipation becomes chronic when it persists for weeks on end.

Constipation may be due to blockages in the rectum, diseases like Diabetes Mellitus etc. However, they could be due to lifestyle issues like not drinking enough water, not eating enough fibre in our diets and being sedentary. Being a woman and getting older also pre-disposes one to more episodes of constipation.

It is possible that the stool is so hard that it has caused a tear of the skin lining the anus (anal fissure, as in your case) leading to the blood seen with the stool.

Prevention is focused on ensuring that you have enough fibre in your diet (fruits, vegetables, whole grain), getting at least 30 minutes of exercise daily and drinking at least 8 glasses of water daily. When you have the urge to poo, try not to delay it for too long. Go and try to take your time to do your business!

How can you help this?

  • Include some fibre in your diet. Fruits and vegetables, oatmeal, local rice and other whole grain meals are helpful.
  • Drinking at least 8 glasses of water daily. Remember that this number, 8, is not cast in stone. It’s just a helpful guide. If you’re in a hot area, you’ll probably drink more
  • Get at least 30 minutes of exercise/activity per day or on at least 5 days of the week.
  • If these do not help, you may want to see your doctor for a proper examination as some diseases like Diabetes Mellitus may also cause constipation.

Overweight and healthy: the concept of metabolically healthy obesity

Carrying too many pounds is a solid signal of current or future health problems. But not for everyone. Some people who are overweight or obese mange to escape the usual hazards, at least temporarily. This weight subgroup has even earned its own moniker—metabolically healthy obesity. Most people who are overweight or obese show potentially unhealthy changes in metabolism, like high blood pressure, high cholesterol, and insulin resistance. But some people who are overweight or obese manage to avoid these changes and, at least metabolically, look like individuals with healthy weights. Such individuals have near-normal waist sizes, blood pressure, cholesterol, and blood sugar, as well as good physical fitness. Metabolically healthy obesity isn’t common. And it may not be permanent.
